Rates starting at $18.90 per hour.We are offering a $6,000 Sign-On Bonus (to be paid throughout the first year of employment).Lutheran SeniorLife provides a comprehensive array of community based health and social service programs throughout western Pennsylvania.Our services include LIFE (Living Independence for the Elderly); skilled nursing, home health; personal care; adoption & foster care; assisted living; adult day services; memory care; hospice; palliative care; residential living; affordable housing; home safety monitoring; Meals on Wheels; rehabilitation services and private duty.LIFE is a program that serves county residents living in Armstrong, Butler, Beaver and Lawrence Counties. It is an all-inclusive comprehensive program that helps individuals maintain their independence in their home and community through supportive services. The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home.Performs basic duties to assist participants in maintaining an environment consistent with their needs. Assists participants in their mobility exercises to help them be more independent for their activities and maintain their current level of functioning. Provides care, across care settings (center or home) in a manner that ensures participant satisfaction and retention. Assists participant in such activities as walking, transfers, use of mobility devises, getting to and from restroom, toileting, providing incontinence care, feeding, showering, getting in and out of transportation vehicles, and socialization at the center. Knowledge and support of the mission and core values of the LIFE Program are important to successful performance, as are commitment to and demonstration of a spirit of teamwork.Under limited supervision and in accordance with all applicable federal, state and local laws/regulations and policies, procedures and guidelines, for this position are:Takes part in participant's assessment and reassessment, including initial and periodic, and the participant's plan of care. Is knowledgeable about participant's overall need and views the participant in a holistic manner. Also attends required meetings and in-services.Provides or assists participants with personal care (e.g., bathing, dressing, toileting, oral and denture care, hair and skin, shaving, nail trimming).Reports to RN Case Manager document changes in a participant's condition according to policy and procedures.Works with other staff members as required in implementing and carrying out services and activities and meeting the needs of individual participants.Assists with day center/multi-purpose room activities, and small group activities as assigned.Assists with dining area set-up and distribution of meals and mid-day snacks, as assigned.Participants in daily Interdisciplinary Team meetings including semi-annual assessment evaluations.Helps participants to move, turn, get in and out of bed, chair or wheelchair, and ambulate with or without assistive devices.Reports all pertinent observations of participants to appropriate medical staff or supervisor.Helps maintain order and cleanliness of the facilities and participant homes, and its equipment and linens.Observes and reports to immediate supervisor any changes in participant's physical or mental status.Escorts participants to specialist appointments, provides assistance and monitoring throughout appointments, and ensures safe return home or to LIFE Program center.On a rotating basis, serves as the leader Personal Care Attendant representative to the daily interdisciplinary communication meetings, sharing pertinent information to all other PCA's for the assigned day.Demonstrates knowledge and appropriate behaviors concerning infection control by proper handling of equipment, soiled clothing, bedpans, urinals and adequate hand washing.Works in a safe manner, following safety guidelines and standards.Attends safety in-services.Offer guidance or assistance to staff working in an unsafe manner.Collects specimens as indicated.Provides or assists participants with non-sterile hot and cold applications as required.Measures and calculates fluid and food intake and output as indicated.Provides or assists with required active/passive range of motion exercises and Restorative Care staff requests.Assists as indicated with home care.Monitors exits and main entrance.Remains flexible and calm in an extremely busy environment.Assists participants who may be confused or suffer from dementia.Maintains a positive and cheerful attitude despite constant disruptions and interruptions.Delivers medications and supplies as assigned.Provides medication reminders.Obtains and records blood sugars as directed.TRAINING: On an annual basis employee will have completed training including:the need of the participants in the center's targeted populationbody mechanics/transfer techniques,voluntary reporting requirements involving abuse, neglect and exploitation,positive approach methods to manage behavior,and two (2) sessions, (totaling a minimum of eight (8) hours), targeted at enhancing the quality of care given and the employees job performance.OPERATING PRINCIPLES....
