He or she assists the clinical and nursing teams in patient care, helps patients by supporting daily living needs, and provides comfort, transportation, and vital sign monitoring.QUALIFICATIONSHigh School Diploma preferredGood understanding of addictive process and recovery skillsMinimum of one-year experience in the addiction field preferredIndividuals in recovery from substance abuse must have a minimum of two years of continuous chemical abstinence and must be active in support groupsPERFORMANCE EXPECTATIONSPerform admissions and orientation of the patient to the unit. Assists with patient dischargesTakes patient vital signs and documents according to facility policyObserves patient behaviors and documents in accordance with facility policyConduct patient check of belongings upon admissionSupervise urine collection of patientsConduct crisis intervention with the supervision of Director of Nursing/ADON or Charge NurseEscort patients to appointments as indicated by supervisor. Accompanies patients on outings and to nightly meetings and may require driving facility vehiclesDocuments in chart any contact with patients in accordance with department and facility procedureServes as a liaison between clinical staff and patients, keeping staff informed of high-risk issues or potential patient problemsRegularly communicates with other team members and interface in a positive constructive and helpful manner to promote cohesiveness, reduce conflict and provide for resolutionMonitors and identifies potential problems and discusses them in the daily multidisciplinary treatment meetingParticipates in meetings and treatment teams as requiredAssists with Crisis Intervention as directed by Director of Nursing or Charge Nurse. Able to understand and react effectively to the unique needs of the patientsMonitors meal times and bathroom breaks (ED Unit only)Follow through with special assignments and responsibilities as requested by the Director of NursingProvides support for activities of daily living by assisting patients with activities they are unable to perform themselvesMaintains patient stability by checking vital signs and weight, testing urine, and recording intake and output informationProvides patient comfort by utilizing facility resources, transporting patients, answering patient questions and requests, and reporting observations of the patient to the nursing supervisorDocuments actions by completing forms, reports, logs, and recordsMaintains work operations by following current policies and proceduresProtects patient privacy and organization's value by keeping patient information confidentialAdheres to professional standards, facility policies and procedures, federal/state/local requirements, and Joint Commission standardsUpdates job knowledge by participating in education opportunities, reading professional publications, participating in professional organizations, and maintaining certification and licensureOther duties as assigned
