Monday through Friday, 8:30a-5p Nursing Facilities - Covers the following zip codes: 40041 40059 40204 40205 40206 40207 40220 40222 40299 CNA Position Summary: The Certified Nursing Assistant (CNA) is responsible for delivery of personal care and minimal homemaker services (i.e. services promoting a safe and clean patient environment) to hospice patients in the home, assisted living facility, or long-term care facility. Services are provided based on Hosparus Health policies and according to the CNA plan of care (POC). The CNA also participates in the development of policies and procedures that guide the provision of hospice care. CNA Position Qualifications: High school diploma preferred. CNA certification required in the state hired to work, or an active LPN or RN license. Valid Drivers License. Auto Insurance coverage. One year home health aide or nursing assistant experience preferred. Use of technology-based tools, including electronic medical record, strongly preferred.
