We offer a wide range of evidence-based rehabilitative services, including mental health and substance abuse treatment, housing, vocational training and job placement, healthcare, education, and creative arts therapies.The Intensive Mobile Treatment (IMT) Team aims to enhance the quality of life for program participants through treatment, rehabilitation, housing, and support services that promote wellness, recovery, community integration, and safety. The IMT team provides and links consumers to these services, guided by principles of access, engagement, continuity of care, and risk awareness. Services include assessment and treatment planning, crisis intervention, medication management, mental health and substance use treatment, individual and group therapies, psycho-education, vocational rehabilitation, medical assistance, housing support, and coordination with the criminal justice and shelter systems.Essential Position Functions:
- Building relationships with program recipients and their networks of support to support the person's recovery.
- Assist individuals with navigating the service systems, including behavioral and medical health, criminal-legal, shelter system, entitlements, and transportation.
- Practice according to the principles of recovery-oriented and trauma-informed service delivery.
- Complete all required documentation accurately and timely.
- Participate in team meetings, including High-Risk meetings.
- Conduct individual sessions with program participants addressing issues that have served as barriers to independence.
- Willingness to share personal, practical experience, knowledge, and first-hand insight to benefit program participants.
QualificationsQualifications:
- High school or equivalent
- Currently certified as a Peer Specialist or Certified Recovery Peer Advocate or certified within a year of hire.
- Bilingual Spanish is highly desired.
