UnityPoint at Home

Chaplain Hospice Visits Fort DodgeFTE: .80 (32 hours a week)Shift: Four 8 hour shift, Monday-Friday, no weekends and no on-call

Primarily responsible for the provision of spiritual care services to meet the social, emotional, and spiritual needs of patients and families/caregivers, either directly or through coordination of care with other spiritual counselors.

Clinical Excellence

  • Facilitates the development of the interdisciplinary team plan of care to meet individual spiritual needs.
  • Maintain record of spiritual care services and related activities for quality assurance, program development, and policies and procedures.
  • Document direct services and ongoing communication with community chaplains, spiritual counselors and hospice interdisciplinary team.
  • Attend Interdisciplinary Team meetings and complete all associated documentation in an accurate and timely manner.
  • Provide consultation, education, and support to the interdisciplinary group on spiritual care.
  • Meets measures as determined and required for job productivity and performance improvement.
  • Provide bereavement follow up services as requested.
  • Arrange for on-call availability of chaplaincy services.
  • Conduct or make arrangements for funeral or memorial services for patients as requested.
  • Assist in planning periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.

Collaboration

  • Develop and maintain a resource group of clergy to whom specific aspects of spiritual care may be delegated.
  • Recruit community chaplains, spiritual counselors, and spiritual care volunteers adequately to meet patient/ family/caregiver needs.
  • Develop community contacts and offer education through congregations about hospice care.
  • Provide educational programs for community clergy, religious and lay representatives as resources allow.
  • Provide educational in-services to the hospice and community.
  • Serves as liaison and support to community chaplains and spiritual counselors.

Performance Improvement

Completion of communication tasks and activities in a timely manner. This would include, but is not limited to: responding to email, voicemail or telephone messages, promptly, accurately, and professionally; attending staff meetings as scheduled or viewing videotapes of those meetings; asking questions of team members and supervisors when needing clarification about various day-to-day issues or patient needs; and reviewing employee communication pieces, to stay aware of UnityPoint at Home programs and initiatives.

  • Participates in quality assurance and performance improvement activities as assigned.
  • Perform other duties as requested by Hospice Administrator or Supervisor to facilitate the smooth and effective operations of the office.
  • Maintain regular and consistent attendance at work.
  • Behave in a manner consistent with Mission, Vision, Values and Expectations for Excellence.
  • Maintain compliance with OSHA, Accreditation Standards and Risk Management guidelines.

Education: Graduate of accredited school of theology, or related Bachelors degree. 2 units of CPE.

Experience: Ordination in a particular religious group and/or endorsed and in good standing from an ecclesiastical governing body.

License(s)/Certification(s):

Valid drivers license when driving any vehicle for work-related reasons.

Valid Mandatory Reporter course completion by state(s) requirement

CPR Certification.

Knowledge/Skills/Abilities:

Proficiency with computer applications (i.e. Excel, Word, Access)

Ability to minister to persons and families in crisis situations

Experience in working with patients/families dealing with life-threatening illness and death.

Must be comfortable in working in an interfaith setting and able to accept different lifestyles, cultures, beliefs, and values.

Strong interpersonal skills.

Ability to work as a collaborative team member across service lines as position requires.

Ability to understand and apply guidelines, policies and procedures.

Able to network with community clergy and congregations.

Possesses knowledge of and commitment to the hospice philosophy of care.
