  • Supervises the nursing assistants and the unit clerk and determines work procedures; prepares work schedules, expedites workflow, evaluates, counsels and disciplines.
  • Ensures that nursing assistants, unit clerk and all supervised personnel adhere to standard job requirements. In the absence of the Nurse Manager, reports any changes in behavior or condition of a Sister in the Primary physician or managed care program.
  • Performs specialized treatments including, but not limited to, oxygen therapy, catheter care, skin and wound care, ostomy care, parenteral fluid administration, enemas, diagnostic preps, venipuncture, sterile technique, traction, cast care and isolation.
  • Makes available emergency measures and fire procedures. Conducts fire drills and scenarios. Participates in educational programs to maintain knowledge of current issues in nursing administration and the practice of geriatric nursing.
  • Oversees and evaluates orientation of all Nurse Aides. Is active in their orientation in regard to approach, attitude and respect.
  • Finds replacements for personnel call-ins to make sure shifts are covered.
  • Provides input into the introductory and annual evaluations of Nurse Aides and forwards this to the Nurse Administrator.
  • Responsible for identifying and appropriately reporting need for disciplinary action in accordance with personnel policies and procedures.
  • Uses the care plan coordinated by the Administrative Team and managed care programs to address the medical, physical, spiritual and emotional needs of the patient. Determines medical care of the patient based on assessments, history, patient's needs and staffing.
  • Oversees and provides the Activities of Daily Living to each patient.
  • Learns use of new equipment and is responsible to train Nurse Aides to use this equipment.
  • Notifies Maintenance Department of any repair needs by use of Maintenance Request Forms.
  • Attends and participates in daily reports, staff and committee meetings as assigned by the Nurse Administrator.
  • Demonstrates knowledge of fire and safety plans, the use of safety devices and conveys this knowledge to staff. Is aware of and follows appropriate universal precautions and isolation protocol.
  • Notifies doctor, Administrative Team and Provincial Councilor and the managed care program where applicable, upon death of a Sister.
Provides quality nursing care to residents in an environment that promotes their rights, dignity, freedom of choice and individuality as illustrated by the following: 40%
  • Assesses residents and analyzes obtained information to develop appropriate interventions to prevent decline; records signs of change in condition; notifies appropriate staff per facility protocol to evaluate weight loss, decline in ambulation, development of skin breakdown and other changes; observes conditions that indicate possible need for restorative nursing programs and then notifies appropriate staff per facility protocol of the need for evaluation; informs the physician of changes in assessment when appropriate.
  • Administers medications and treatments to residents; monitors administration of intravenous fluids; places orders for medication with the pharmacy.
  • Responds to Nursing Assistant and other Staff requests in performing physical care and for assistance in assuring and evaluating residents.
  • Assesses residents on admission to develop interim care plan and further assesses in depth to contribute to the complete care plan. Updates care plans as warranted and required. Attends and participates at Plan of Care meetings as scheduled.
  • Responds to emergencies; assesses the condition of the resident; decides on the type of medical attention required; calls appropriate ambulance service if needed; notifies family when resident is transferred or admitted to the hospital; notifies the physician when a change in condition occurs.
  • Confers with residents and healthcare proxy and makes referrals when appropriate.
  • Maintains resident clinical files; keeps charts updated; documents.
  • Completes employee and resident incident report forms; completes follow-up investigation forms.
  • Makes rounds to see each resident daily and as needed.
  • Ensures that residents are aware of the services available to them; encourages residents to participate in activity programs and provides staff support to enable residents to get to and from activities and to participate in them.
  • Maintains accountability for controlled substances; inventories medications at the beginning and end of the shift with another licensed nurse; notifies supervisor of discrepancies.
  • Supports the facility's medical supply system;
  • Communicates status of residents to incoming personnel according to established procedures; communicates with physicians; transcribes, clarifies and implements physicians' orders.

  • Assist patients, family members or other clients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.
  • Answer and refer telephone calls or other inquiries to ensure accurate and timely communications are facilitated.

QUALIFICATION STANDARDS
  • Valid registration as an RN/LPN in NYS.
  • Proficient reading, writing, grammar, and mathematics skills.

  • Knowledge of drug interactions, side effects and incompatibilities.
  • Decision-making skills.
  • Proficient assessment skills.
  • Basic typing skills.
  • Proficient with computer work.
  • Auditory and visual skills.
  • Ability to sit, stand, lift, bend, reach and push and pull items weighing 50 pounds or less
  • Graduate of an accredited school of Nursing.
  • Current CPR certification.
  • One year of experience in similar setting/healthcare in similar capacity (preferred).
ERGONOMIC REQUIREMENTS Sitting, standing, stooping, bending, pushing, heavy lifting (50 pounds), carrying. All of the ergonomic requirements must be performed without significant risk of injury to oneself or others, or otherwise demonstrate or explain how one can perform essential functions listed above. This job operates in a nursing facility setting. This role also could come into contact with residents who may have contagious illnesses. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. This is a position in a health care setting that is open 24 hours a day, 365 days a year. Rotating shifts as well as holiday and work during inclement weather may be required.DRESS CODE
  • Any article of clothing that is deemed appropriate healthcare attire such as scrubs is acceptable.
  • Footwear: comfortable close-toed shoes or sneakers, no sandals.
  • Makeup must be tasteful and modest.
  • Nails must be at a modest length;
  • Nose rings or facial piercings are limited to posts, no hoops. Modest post or small hoop earrings only, short necklaces, watches and bracelets are also permitted.

  • Staff is always expected to be clean and well-groomed and ready to care for our Sisters
