Ideal candidate will be eligible for a $15,000 (less taxes) sign on bonus.

RiverSpring Certified Home Health Agency provides the acutely ill patient with short term skilled home visits: nursing, physical therapy, occupational therapy, speech therapy, and social work.

Responsibilities:

The CHHA home care RN is responsible for clinical assessments, development, oversight, and implementation of the approved patient care plan as developed by the patient/family and physician to maintain the patient safely at home. S/he will ensure that the care plan is appropriate and responsive to the patients health care needs. S/he will reevaluate clinical assessments and make any necessary changes to the care plan. Provides oversight of Home Health Aide services and participates in the interdisciplinary care delivery process for all patients admitted to the CHHA. This position requires the individual to work at least one weekend per month. Covering Bronx, Lower Westchester or Manhattan.

Qualifications:

Current New York State Registered Nurse License required and a Bachelor Degree in Nursing or equivalent applicable education and experience required. Masters degree preferred. Two (2) years experience assessing patients for home care safety and developing appropriate care plans and plan adjustments required. Valid drivers license required. Must be able to read, write, and speak English. Ability to read, analyze, and interpret general business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. Ability to work with mathematical concepts such as probability and statistical inference, and fundamentals of plane and solid geometry and trigonometry. Ability to apply the nursing process and evidence-based practices to care delivery.

Compensation: $105,000 min. to $110,000 max. Annually
