Job Summary:

The role of a CHHUNY Care Manager is to be the key point of contact in managing children & youth services. The Care Manager will be the center point of the communication to ensure all providers are working together with the child/youth's best interest in mind. The Care Manager will conduct comprehensive assessments in order to obtain the most accurate information to build off of the identified needs and risk factors.

Minimum Qualifications: Health Home Care Managers that serve High acuity enrollees must have the following minimum education and experience:

• A Bachelors of Arts or Science with 2 years of relevant experience, or
• A License as a Registered Nurse with 2 years of relevant experience, or
• A Masters with one year of relevant experience.
