Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Job DescriptionUnder the direction of the Senior Vice President (Post-Acute/Long Term Care), or other members of senior leadership, plans, organizes, directs, and manages facility operations, and implements policies and procedures. Responsibilities include, but are not limited to, making operational decisions, ensuring financial sustainability by achieving annual facility budget, providing general supervision, programming, and on-going evaluation of care and services, ensuring the health and safety of residents, patients, visitors, and staff, and compliance with all applicable local, state, federal and NYC Health + Hospitals rules and regulations and standards of care, to improve outcomes and the patient experience.
  • Plans, develops, and communicates programs, policies, procedures, and evaluative and monitoring techniques to implement policies and achieve goals established by senior leadership.
  • Directs and coordinates all staff and support services activities, ensuring proper monitoring, and identifies and capitalizes on opportunities to streamline, standardize, and centralize (i.e., share) operations and services to maximize efficiency and effectiveness.
  • In partnership with facility leadership, directs the implementation of health care system programs and policies concerning facility operations, and coordinates activities between facility staff and senior leadership; ensures that required programs are consistent with the priorities and standards of senior leadership and NYC Health + Hospitals.
  • Ensures ongoing compliance with regulations and standards established by accrediting agencies and regulatory bodies, including the New York State Department of Health and the Joint Commission, and initiates and coordinates compliance measures, as required.
  • Selects and appoints department heads and establishes an organizational structure that delineates authority, responsibility and accountability and includes internal control mechanisms; formal systems of financial accountability; retention of knowledgeable personnel; and scope of clinical services, in collaboration with clinical and system-wide leadership.
  • Collaborates with finance, facility medical, administrative and operations staff in the planning, development, and maintenance of an annual budget.
  • Authorizes purchases of supplies and equipment, in accordance with policies governing purchasing procedures, product selection and evaluation, quality and health care needs, to ensure optimal service outcomes.
  • Undertakes construction and renovation programs including acquisition and replacement of facility equipment.
  • Schedules, conducts, and attends regular interdepartmental and departmental meetings; designates facility department representatives to medical staff and multi-disciplinary committees, to facilitate effective communication within the facility.
  • Develops, maintains, and implements a facility management reporting system that provides adequate and timely data for program planning and evaluation purposes.
  • Operates facility as part of a health care system, implementing system-wide decisions in a timely fashion and without deviation.
  • Demonstrates effective business/strategic planning within health care system framework; identifies growth/contraction opportunities, in context of community needs; and establishes high performance expectations.
  • Meets budget/key performance indicators (KPIs)/metrics of success, and manages resources effectively.
  • Establishes clear roles/responsibilities for staff, provides clear direction, engages employees, and builds trust.
  • Maintains transparency by communicating, asking for assistance, and informing Central Office of significant issues.
  • Engages clinical/physician leadership in focusing on clinical quality.
  • Engages community leaders, builds trust, and advocates for our services.
  • Maintains cooperative relationships with the community, professional groups, external agencies, unions and regulatory bodies; integrates services of the facility with the community's resources, as appropriate.
  • Performs other related duties in support of the strategic goals of the system, as required.
Minimum Qualifications
  • Valid New York State license and current registration to practice as a Nursing Home Administrator issued by the New York State Department of Health; and,
  • A Master's degree from an accredited college or university in Health Care Administration, Business Administration, Public Health Administration, Long Term Care Administration; or related field; and,
  • Five (5) years of paid, full-time experience in health care administration or management, above the department-head level utilizing a thorough knowledge of skilled nursing facility administration, regulatory compliance pertaining to skilled nursing facility operations with special reference to accreditation procedures; or,
  • A satisfactory equivalent of education, training and experience. However, all candidates must have at least a Baccalaureate Degree or higher, and have a valid New York State license and current registration to practice as a Nursing Home Administrator issued by the New York State Department of Health.
