Job Description:Independence Association is seeking a full-time, energetic case manager to join our dynamic children's case management team. Our case managers play a key role in assisting individuals with intellectual and developmental disabilities to access resources and live full and inclusive lives. They work closely with both the individual and their identified support group. Our case managers provide thorough assessments of strengths and needs, comprehensive planning of services, advocacy, outreach, and resource coordination unique to every person served. Manage a caseload assigned by team leader and/or program manager.Ensure that consumers are seen in-person in accordance with their DHHS contract and that office time is sufficient to complete all follow up activities identified in the ISP including but not limited to referral, documentation, advocacy telephone contacts, electronic communication, and monitoring of services.Enter all notes of service into the agency electronic medical records system within established agency timelines.Complete comprehensive needs assessment at least annually and more frequently if needed.Coordinate, schedule, facilitate and/or lead the members of their team to create the ISP. Once prepared, manage the on-going documentation and monitoring of the ISP.Provide linkage to services to meet identified barriers through referral, advocacy, and monitoring of services in accordance with MaineCare policy.Assist individuals and their families to make choices, achieve personal goals, and promote the individual's inclusion into the community as identified in the ISP.Maintain a billable productivity level established by the agency.Coordinate and facilitate transition and discharge planning.Assist and monitor individuals' efforts to secure and maintain needed services and entitlements such as housing, financial benefits, health needs and educational supports.Support individuals and their families in crisis to access needed resources.Advocate for individuals alleging rights violations.Comply with all agency policies and procedures, particularly as they relate to case management and its relationship to the standards and requirements of accrediting and funding sources.Travel to client's home or community setting.File maintenance. Requirements Bachelor's degree (required) Experience of 1 year in human services field (required) Valid driver's license (required) Ability to perform under limited supervision Great written and oral communication skills Comfort advocating for individuals with various levels of disability in a variety of settingsPhysical Requirements*:Ability to drive a vehicle for up to a half an hour commute to the home of family.Ability to type on the computer for up to 4 hours per day with breaks.*Reasonable accommodations may be made to enable individuals with disabilities to perform essential job functions.Salary Description $45,760 - $51,450
