Salary Range $20.00 - $24.00 Per HourPay within this range is based on experience and education DEI Values Statement:Mary Cariola Center recognized that a high performing organization is one that cultivates a culture where everyone feels like they belong.Through continued dialogue, education, a policy development, we are committed to integrating Diversity, Equity, Access and Inclusion into all aspects of our work.As an agency found on principles of inclusion, Mary Cariola Center fosters an environment where all are welcome, and our differences are respected and valued.ESSENTIAL DUTIES AND RESPONSIBILITIES Sensitivity to the cultural diversity of othersUnderstands the medical, psycho-social, educational characteristics and needs of the population served by the AgencyThrives working independently and on a teamProvides outreach and enrollment, develops Plans of Care and Individualized Family Service Plan (IFSP), and conducts continuing care management services with children and familiesParticipates actively in trainings, meetings, and functions at the agency and in the communityProvides quality direct Care Management services to children and families in home, school, and community settingsUses a strength-based approach to arrange for, educate about, and manage the coordination of medical care and other services as neededProvides on-call availability to Health Home Program as assignedInterfaces and networks with partnering Health Homes, providers, and agenciesMust obtain certification to administer CANS-NY within 30 days of employment; certification must be continuously maintained while in the positionProvides transportation to children and /or families to appointments (i.e. medical appointments, Plan of Care meetings, DHS, community agencies, etc.) as needed.
