POSITION FUNCTION/OVERVIEW:This position is responsible for full oversight of each patient's Continued Care Plan by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress in order to provide a satisfactory client experience. The Case Manager will work with the Director of Case Management to ensure all policies and procedures regarding discharge planning, and case management service needs are in compliance. The case manager will offer continuous client care, and support with discharge planning/referrals. PRIMARY DUTIES/RESPONSIBILITIES:
- Promote the mission, values, vision, and philosophy of organization.
- Provide excellent customer service for clients; practices confidentiality and privacy protocols in accordance with HIPAA requirements.
- Understand and implement individual treatment plans, record and document relevant information regarding the client's behavior and activities.
- Creates, develops, communicates, and completes a continued care plan with the patient ensuring the highest level of recovery opportunity upon discharge.
- Document any and all notes into EMR accurately and complete as it pertains to interaction with patients and the communication of their discharge plan and aftercare schedule.
- Provide assistance with reviewing referral options and coordinating assessments with outside organizations.
- Ensure all pertinent ROI are obtained and signed by the patient and uploaded into CRM.
- Provide a safe, supportive environment for the client.
- Participate in crisis intervention situations.
- Maintain the utmost level of professionalism with clients at all times, refraining from any personal conversations or relations.
- Responsible for completing initial assessments, assisting individuals with legal issues, FMLA, Food Stamps, coordinating external outside appointments, discharge planning, MAT (Medication Assisted Treatment), etc.
- Maintains established Company policies and procedures, objectives, quality assurance program, and safety, environmental and infection control standards.

QUALIFICATIONS REQUIRED:
- High School Diploma or GED equivalent with combination of education and work experience, required; Associates Degree or Bachelor's Degree, preferred.
- Minimum of one (1) year experience in the behavioral healthcare industry.
- Must be available evenings, weekends and holidays, as needed.
- Ability to work flexible schedules and shifts, required.
- Relevant computer software and hardware applications proficiency - Word, Excel, PowerPoint, Outlook, Electronic Medical Records, Billing Systems and/or other scheduling applications; KIPU preferred
