Our mission is centered around creating a nurturing environment that upholds independence and dignity, with a keen focus on addressing the unique needs of each client through a consultative and caring approach.Your Role as a Client Care Coodinator:Engage directly with Seniors and their families to understand their specific care needs, crafting tailored solutions for their safety and independence at home.Maintain empathetic and effective communication with seniors and their families, ensuring their needs and concerns are addressed with care and understanding.Serve as a resource to internal employees, clients and community members, creating connections and offering solution based recommendationsDiligently document interactions, follow up on commitments and complete tasks promptly to maintain high standards of serviceCollaborate effectively within a team setting while also thriving independently to achieve optimal outcomes for our clients.Utilize various software tools to enhance operational efficiency, facilitate seamless team communication and uphold our commitment to quality care delivery.What We are Looking For:BS Degree or relevant coursework in Gerontology, Social Work or Health Care Administration; MS Degree in related field a plusMinimum of 2-3 years prior work experience in a service oriented role that includes face-to-face customer interaction and problem resolutionDemonstrated passion for helping othersPrevious experience in senior care or related field (Hospice, Home Health, Hospital, Skilled Nursing, Rehabilitation Facility) is desirable.Excellent verbal and written communication skills, with a demonstrated proficiency in using a consultative approach communication styleStrong documentation skills with a focus on thorough follow-up and follow-through on all interactionsAbility to excel in a fast-paced environment, both as an individual and as part of a dynamic teamProficiency in software programs for operational efficiency, communication and documentationDemonstrated effective time management skills What Sets You Apart: The ideal candidate is a compassionate, solution-oriented individual with a deep commitment to and passion for improving the lives of seniors.
