Position Overview:The Medical Client Care Coordinator is responsible for managing and coordinating the care and services provided to patients within the medical facility. This role involves ensuring that patients receive timely appointments, accurate information, and support throughout their care journey. The Client Care Coordinator will serve as the primary point of contact for patients, work closely with healthcare providers, and manage administrative tasks related to patient intake, scheduling, medical records, and insurance.
Key Responsibilities:
Patient Intake and Coordination:
- Serve as the initial point of contact for new and existing patients, ensuring that all necessary forms and documents are completed accurately.
- Collect and verify patient information, including medical history, insurance details, and personal data.
- Maintain a welcoming and professional atmosphere, addressing patient inquiries and concerns in a compassionate manner.
Appointment Scheduling and Follow-Up:
- Schedule patient appointments with healthcare providers, ensuring that appointments align with both patient and provider availability.
- Confirm patient appointments via phone, email, or text and send reminders prior to scheduled visits.
- Reschedule appointments as necessary and handle cancellations to ensure optimal scheduling efficiency.
Patient Communication and Education:
- Act as a liaison between patients and healthcare providers, communicating patient needs, treatment plans, and appointment details effectively.
- Educate patients on healthcare services, procedures, and treatment options, helping them make informed decisions regarding their care.
- Provide guidance on pre-appointment instructions, such as fasting or medication adjustments.
Insurance Verification and Billing Assistance:
- Verify patient insurance coverage and inform patients about their financial responsibilities, including co-pays and deductibles.
- Assist patients with understanding insurance plans, benefits, and claims processes.
- Work closely with the billing department to resolve billing issues and ensure accurate processing of claims.
Medical Records Management:
- Maintain accurate and up-to-date electronic health records (EHR) for all patients, ensuring that all information is complete and confidential.
- Update patient records with new information, treatment details, and follow-up care as directed by healthcare providers.
- Ensure compliance with HIPAA regulations and protect patient privacy in all communications and record-keeping.
Care Plan Coordination:
- Support the coordination of patient care plans, working with healthcare providers to ensure that treatments and services align with patient needs.
- Assist patients with understanding their care plans, medication schedules, and follow-up appointments.
- Ensure that patients are informed about any necessary changes to their treatment or healthcare services.
Problem Resolution and Patient Support:
- Address patient concerns or complaints in a timely and professional manner, ensuring a positive patient experience.
- Escalate complex or unresolved issues to management or appropriate departments when necessary.
- Provide emotional support and empathetic communication to patients navigating medical care.
Compliance and Quality Control:
- Ensure all activities comply with relevant healthcare regulations, including HIPAA and other federal, state, and local laws.
- Assist with audits and reviews of patient files to ensure that documentation is accurate, complete, and meets quality standards.
- Stay up-to-date on industry best practices and changes in healthcare regulations to ensure compliance and high-quality patient care.
Collaboration with Healthcare Team:
- Work closely with doctors, nurses, medical assistants, and other healthcare providers to ensure that patient care is coordinated effectively.
- Communicate patient information, treatment updates, and changes in care plans with the healthcare team to ensure that patients receive seamless care.
- Participate in team meetings and case discussions to help ensure the best outcomes for patients.
Qualifications:
- Education: High school diploma or equivalent required; Associate's or Bachelor's degree in healthcare administration, medical assisting, or related field preferred.
- Experience: Previous experience in a medical office, healthcare setting, or client care coordination role preferred.
- Skills:
- Strong organizational and multitasking abilities.
- Excellent communication skills, both verbal and written.
- Empathy and a patient-centered approach to care.
- Knowledge of medical terminology, insurance processes, and EHR systems.
- Ability to handle confidential patient information with discretion and professionalism.
- Proficiency with Microsoft Office Suite and other office software.
- Familiarity with HIPAA regulations and healthcare compliance standards.

Work Environment:This role typically operates in a healthcare or medical office environment, with regular interaction with patients, healthcare providers, and administrative staff. The position may require occasional overtime during peak hours.
