Job descriptionPrimary Duties:
- Work in collaboration with Clinical Supervision on aftercare needs including appointments as deemed necessary by Clinical Supervisor/staff.
- Obtain consents for all aftercare resources.
- Complete all Continuing Care Plans for client discharge as well as provide resources for discharge.
- Complete tasks assigned and within your scope of practice.
- All other duties as assigned.
Education / Experience:
- Minimum of High School Diploma/GED
- 1 year of working in the Substance Abuse field
- 1 year of case management
Skills required:
- Demonstrate proficiency in communication, observation, written and interpersonal skills.
- Current and valid Drivers License.
- Knowledge of addiction.
- Communicate effectively with patients, families, and outside referral agencies.
- Maintain accurate documentation and records in the client record.
- Ability to perform functions of CMA in substance abuse setting.
Physical Requirements:
- Prolonged period of time on your feet .
- Must be able to lift up to 50 pounds.
