We are an employer of choice for anyone interested in a meaningful career in the social services field.The Patient/Client Navigator reports to the Director of Social Services and assists the Director of Social Services in ensuring that clients are connected to medical care, enrolling for medical insurance if/when applicable, and participating in ongoing medical care.RESPONSIBILITIES
- Assist shelter residents with benefits access, health insurance, navigating the healthcare system, and securing permanent housing.
- Assume the role of a Housing Specialist to assist families with navigating through the housing process and moving into permanent housing.
- Documents client-specific engagement and activities in designated electronic record within a timely fashion (CARES)
- Seek community resources that are applicable to the client population, update the community resource binder.
- Seek and create relationships with landlords and brokers that work with various city/state programs such as City FHEPS and SOTA.
- Escort clients to housing viewings and apartment walkthroughs.
- Collaborate with agencies/providers for any workshops or drives that would be beneficial to our clients.
- Participates in program meetings and trainings.
- Prepare and submit housing applications and rental packages utilizing systems such as Current, HOME, NYCHA portal, etc.
- Provide the list of FQHCs and H+H locations as provided by DHS
- Assist all clients and families in making primary care appointments, prenatal appointments, etc.
- Provide directions and transportation assistance if needed (including escorting clients to their appointments)
- Assist clients and families in enrolling in health insurance or H+H NYC Care
- Assist clients in applying for HRA or any other benefits they may be eligible for, including assisting them with calling said agencies, submitting documents, and escorting clients to offices if in-person assistance is required.
- Track status of their cases, submit denial/rejection information to DSS and RD for client advocacy.
- Manage a caseload of migrant families and maintain a master list of their benefit, insurance, and housing status.
- Any other responsibilities that are assigned by DSS or RD.
QUALIFICATIONS
- Associate's Degree Needed, Bachelor's Degree Preferred.
- Strong computer skills including proficiency with MS Excel or other spreadsheet program.
- Strong writing and communications skills.
- English-Spanish language proficiency.
PHYSICAL REQUIREMENTS
- Ability to use a computer for prolonged periods.
- Ability to occasionally lift and/or move up to 20 pounds.
- Ability to stand, walk, or sit for long periods of time.
- Ability to climb five flights of stairs
- Ability to bend and retrieve objects and/or documents.
- Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
- Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather.
