ARC is dedicated to unifying the region's collective resources to prepare the metropolitan area for a prosperous future. This is done through professional planning initiatives, the provision of objective information, and the involvement of the community in collaborative partnerships. As a member of the Innovations Team, the Client Services Liaison (Behavioral Health Coach) will provide person -centered coaching and health education to assigned clients and link them to resources and services that address mental and physical health needs so they can remain in their community. This individual will also utilize a person-centered approach to address barriers to social determinants to improve health outcomes and includes complex care coordination efforts across teams to address health conditions such as behavioral health, diabetes, and other chronic health issues. This role is remote and field-based and will include performing in-home consultation/assessment and care plan development for persons with behavioral health needs. Essential Duties and Responsibilities:
  • Schedule and conduct face to face (in person) or telephonic consultation and provide coaching support to assigned clients and/or their caregivers;
  • Meet with individuals and/or caregivers to complete needed assessments/documentation;
  • Determine client's unmet needs, barriers, preferences, and goals, while applying person centered principles; develop and implement the most appropriate action plan;
  • Conduct individualized resource searches and identify appropriate resources to meet the established needs;
  • Make referrals and link clients and/or caregivers with the most appropriate resources;
  • Conduct needed follow-up and responds to client requests;
  • Communicate with community and health systems partners as needed;
  • Maintain spreadsheets, databases or reports used to share outcomes and opportunities with management or external payors;
  • Present programs and progress to partners and other professionals as needed;
  • Complete reports with required documentation and prescribe contacts within required time frames;
  • Serve as a liaison with organizations and businesses in the aging services community; and
  • Perform other duties as assigned to support agency goals and objectives.
Minimum Qualifications:
  • Bachelor's degree in social work, human services, gerontology or related field;
  • One year of related counseling or client advocacy or information/referral experience in area of assignment (such as mental health, aging or disability services)
OR
  • An equivalent combination of education and experience sufficient to successfully perform the essential duties of the job such as those listed above, unless otherwise subject to any other requirements set forth in law or regulation.
PREFERRED QUALIFICATION:
  • Master's degree in social work, gerontology or related field
SPECIAL REQUIREMENTS:This role is field based after introductory period and reliable transportation is required.THE SALARY RANGE THAT THIS POSITION WILL BE HIRED AT WILL BE $60,205 - $81,276. (no SS deductions except for Medicare portion, approx. 1.45% of salary) Required Knowledge, Skills, Abilities and Competencies:
  • Knowledge of the behavioral health, substance abuse and long-term care services network;
  • Knowledge of Medicare and Medicaid eligibility and social service resources for aging population served including networks and community resources;
  • Knowledge of Elderly and Disabled Waiver and other Medicaid waiver programs;
  • Ability to understand and practice active listening techniques;
  • Ability to establish rapport with clients to elicit needs and match with appropriate resources;
  • Ability to exercising confidentiality and maintain records and files;
  • Ability to organize task, meet deadlines, and manage and prioritize competing demands;
  • Skill in providing attention to detail in assignments;
  • Possess exceptional time management, interpersonal and communication skills;
  • Possess strong collaboration and partnership development skills with internal and external partners;
  • Proficient in using database technology and Microsoft Office Suite (Outlook, Word, Excel, Teams and PowerPoint); and
  • Ability to manage and prioritize complex competing priorities.
