We are looking for a motivated and energetic person to join our Psychiatric Rehabilitation team. Our Client Support Specialists' provide person-centered support in a team environment to clients both on-site at the program and in the community. The focus is on teaching skills both individually and in groups and accessing resources for resiliency, recovery and independent living and success in life. It includes assessing consumers' needs and strengths, assisting with development of the rehabilitation plan, assessing progress, and maintaining the clients' medical record. Also included is providing needed transportation and meeting productivity measures and daily documentation requirements.If you are passionate about working to help others reach their goals/overcome obstacles and have an interest in Behavioral Health, this is the perfect position for you!
  • Assist clients in assessing their strengths, needs, resources and desires and in developing their Individual Rehabilitation Plan (IRP) as frequently as required or needed.
  • Coordinate services provided by Crossroads for assigned clients.
  • Assist clients in meeting their goals and assess and document their progress each month.
  • Assist clients to develop resources needed to promote recovery and resiliency and life skills necessary to maintain independent living and success in life.
  • Maintain clients' medical records in accordance with regulatory and accreditation requirements.
  • Utilize computerized database(s) or other procedures as required to submit accurate, timely and professional documentation of services in accordance with agency policies, regulations and accreditation standards.
  • Routinely collaborate with family/caregivers as allowed and other providers of services to clients.
  • Provide transportation to clients when needed.

  • High School Diploma or equivalent.
  • Credit given to AA, BS in Behavioral Health, Psychology, Social Work, or related field and 2 years of positively referenced work history in human services field.

Requirements:
  • Computer skills sufficient to enter the required data and documentation.
  • Pass the agency's background check.
  • Clean driving record to support insurability by the agency's insurance carrier.
  • Valid driver's license and ability to drive legally and safely transport consumers in own vehicle if needed.
  • Work related use of a vehicle carrying minimum of 100/300/50 liability insurance.
