The care, support and education patients receive from their primary care team includes everything from preventative screenings and exams for all ages, care of everyday health problems and issues, as well as the care and coordination needed for any specialty concerns.Mission for LPN/CMALPN/CMA Clinic will provide personalized, quality care and care coordination for clinic patients in collaboration with their assigned provider.Position SummaryAs an LPN/CMA, you will be an integral part of our clinical team, as well as have the opportunity to form connections and make a direct, positive impact on our patients and team members. This role performs a variety of professional nursing skills that provide and enhance patient care. Day to day activity includes reviewing and prepping for upcoming office visits, completing office visit rooming tasks which includes obtaining and recording vital statistics and patient quality measures. The LPN/CMA will assist or complete standard clinic procedures along with follow up needs including call backs and relaying of provider recommendations. They will engage with peers and offer assistance as needed.
