Job Description:
Under direction of the Clinical Director of RHCs, the Clinic Nurse is responsible for the provision of patient-centered, evidence-based care, which includes assessment, nursing diagnosis, outcome identification, planning, implementation, and evaluation. The Clinic Nurse participates in decision making, teaching, mentoring, and collaboration with the multidisciplinary team to advance the patient's plan of care, achieve outcomes, and enhance operational efficiency. Essential Duties and Responsibilities:
  • Performs nursing and administrative duties and assists the provider as part of the clinical care team.
  • Conducts pre-visit planning by reviewing and updating patient charts prior to appointment with recent test results and correspondence and determines needed services per practice protocol.
  • Triages and responds to patient communications within practice policy.
  • Confers with provider regarding any incomplete patient tests/consults or other incomplete orders prior to patient visit.
  • Prepares patients for examination and treatment escorting them from the waiting area to the exam room and taking histories and vital signs as needed.
  • Coordinates care with disease management or case programs as needed.
  • Prepares, cleans and maintains exam and treatment rooms as needed.
  • Gives injections and draws blood; assists with EKGs, OB/GYN exams and office procedures as needed; provides other patient care services as directed by the provider.
  • Prepares and maintains clinical supplies and equipment.
  • Assists with scheduling tests and treatments, and processing referrals to other providers.
  • Participates in professional development activities and maintains professional affiliations.
  • Creates and maintains patient records and other information as needed.
  • Interacts with patients, families, staff, and providers in a professional, positive, and respectful manner.
  • Demonstrates a strong commitment to patient-centered care within an interdisciplinary environment.
  • Consistently demonstrates a positive attitude.
  • Fosters teamwork by offering assistance to others.
  • Demonstrates clear ownership of workplace and patient safety.
  • Reports mistakes, near misses, adverse events and quality and safety concerns.
  • Participates in the development and implementation of safety and quality improvement activities.
  • Protects patient confidentiality and abides by all HIPAA and privacy laws.
  • Performs other related duties as required.
Job Qualification Requirements:Education: Graduate of an accredited Licensed Practical Nursing program or an accredited Registered Nursing program.Licensure/Certification: Current Licensure through State of Indiana required. CPR certification required.Experience: One to three years of clinical experience required. Knowledge of medical equipment and instruments, common safety hazards and precautions and treatment room procedures required. Ability to maintain strict confidentiality.
