Job Description:
Job PurposeCollaborate with multidisciplinary team members to deliver appropriate nursing care relevant to recovery from and prevention of substance use disorders, to include (but not be limited to) general health and nutrition education, screenings for metabolic disorders and infectious diseases, administration of injectable medications for mental health and substance use conditions, and wound care. Qualifications
  • Must be a licensed Registered Nurse (RN).
  • Psychiatric/Behavioral Health nursing and healthcare technology experience preferred.
  • Ability to perform basic nursing assessments.
Core Job Duties
  • Guide patients through recovery and clinical services across organizations, reducing barriers to access and improving adherence to care.
  • Ensure effective implementation of MAT and monitor outcomes.
  • Advocate for individuals' health needs, addressing barriers such as stigma, transportation, financial, and insurance challenges.
  • Collaborate with the Carter County BJA Grant Case Manager and Peer Support Specialist to deliver holistic, integrated care for individuals navigating the justice system and mental health services.
  • Ensure the provision of a continuum of care, working with emergency departments, recovery housing facilities, and other community organizations to support long-term recovery and stability.
  • Track and submit grant-relevant program and project-related data to the Project Coordinator at the Carter County Courthouse as requested.
  • Build connections with community resources and recovery housing facilities to support patients' long-term recovery and stability.
  • Participate in team meetings and case reviews to align healthcare, legal, and recovery efforts, fostering seamless communication across the coordinated care team.
  • Take person(s)-served vital signs; perform metabolic screenings for person(s) served.
  • Manage written prescription(s) documentation; manage medication injection(s) and supporting documentation.
  • Manage Patient Assistance for all medications.
  • Complete prior authorizations for Medicare/Medicaid/insurances.
  • Actively respond to communications regarding medications for person(s) served and notify Prescribers of adverse reactions.
  • Handle request for records/discharge summaries as requested.
  • Administer Tuberculosis (TB) testing for staff.
  • Maintain medication list for new person(s) served.
  • Actively work refill requests from pharmacies.
  • Perform screenings as requested; perform blood/urine collection(s) as requested.
  • Request lab results and follow up with reported results.
  • Call in prescriptions and refill medications as needed; maintain sample medication orders.
  • Weekly updates on sample medication inventory and storage as well as quarterly inventory report.
  • Maintain list of discharged person(s) served and medication refills provided.
  • Adherence to all applicable evidence-based practice models.
  • Orally communicate information effectively and accurately.
  • Assure program operates within the constraints of the agency's certification (CARF, DMH) standards and strategic plan related to clinical care.
  • Abide by program and agency policies and procedures.
  • Other job duties and special projects as assigned.
