POSITION SUMMARY:
To be fully engaged in providing Quality/No Harm, Customer Experience, and Stewardship by providing exercise assessment and planning and cardiac rhythm evaluation. Under the supervision of the Medical Director, he/she is trained to oversee prescribed cardiac rehabilitation treatment. He/she is responsible, as a member of a multi-disciplinary team for promoting optimal outcomes. This person demonstrates an understanding of the philosophy of quality-driven, patient-centered care. Along with the Exercise Physiologist, the RN will continuously monitor patients for exercise tolerance and adverse response to exercise.
PRIMARY ACCOUNTABILITIES:
Quality/No Harm:
* Adhere to Health First Cardiac Rehabilitation policies and procedures in accordance with accepted standards of practice, i.e. FL Practice Act, AACVPR Guidelines, Joint Commission and Medicare.
* Participate in the orientation and initial assessment of new cardiac rehabilitation patients. Contribute to the development of the Individualized Treatment Plan with team members.
* Assess patients' risk factors and demonstrate ability to offer support and develop strategies to aid patients in successful therapeutic lifestyle changes.
* Thorough documentation of initial assessment findings, treatment session results, and reassessment outcomes, addressing clinical and behavioral data in the medical record.
* Ensure all components of the Individualized Treatment Plan are completed and certified by medical director or referring physician every 30 days for the duration of the program.
* Immediately respond and report any new or changing patient conditions to supervising and/or referring physician.
* Contribute expertise as a member of an interdisciplinary care team promoting cardiac rehabilitation outcomes by demonstrating independent judgment and anticipate needs of cardiac rehabilitation patients.
Customer Experience:
* Ensure all cardiac rehabilitation patients and family/friends demonstrate an understanding of the Individualized Treatment Plan.
* Routinely review plan of care and goals with patients and family/friends at each treatment session to enforce the importance of inclusion and value of input from patients and family/friends.
* Complete Focus on Therapeutic Outcomes at timely intervals to assess overall satisfaction with cardiac rehabilitation.
* All staff will ensure every patient is treated with courtesy and respect. Staff will present with a caring and profession tone of voice and behavior when addressing personal goals.
Stewardship
* Demonstrate timely management of schedule for smooth throughput of cardiac rehabilitation assessments, re-assessments, and treatment sessions.
* Weekly review of individualized treatment plan to address progress and/or modifications.
* Demonstrate efficient use of supplies and maintenance of equipment to promote life span.
* Ensure preventative maintenance on equipment and report any issues.
MINIMUM QUALIFICATIONS:
* Current Florida RN license, BS required,
* Current BLS and ACLS Certification
* Knowledge of exercise equipment and procedures unique to Cardiac Rehabilitation
* Experience in ECGs and Telemetry knowledge or complete course within 3 months of hire
PREFERRED QUALIFICATIONS:
* MS
* Certified Cardiac Rehabilitation Professional (CCRP); may consider seeking certification within 6 months of hire from the American Association of Cardiovascular and Pulmonary Rehab (AACVPR)
PHYSICAL DEMANDS:
* Able to meet physical demands of: standing, walking, stooping, bending, kneeling, carrying and pushing/pulling. Exerting 20-50 pounds of force occasionally, and/or 10-25 pounds of force frequently.
* Demonstrate role modeling by practicing a healthy lifestyle to cardiac rehabilitation program customers.
MENTAL DEMANDS:
* Must be able to interact with a variety of customers, i.e. physicians, patients, family/friends, insurance payers etc.
* Must be capable of performing multiple tasks in a group treatment setting, maintaining a positive attitude.
