We are hiring a full-time Clinical Admission Liaison to join our dynamic care team at Cape Regency Rehabilitation & Health Care Center in Barnstable! The liaison is responsible for building referral relationships with area hospitals and other partners including onsite evaluations of all potential residents. Education and Experience:
- Healthcare experience required.
- RN, LPN, and/or social work experience preferred.
Duties and Responsibilities:
- Performs onsite evaluations as requested by the facility in a timely manner in order to coordinate admissions.
- Is readily available to represent the facility to potential patients, families, hospitals and other referral sources in a positive, honest, and professional manner.
- Communicates all available patient information (medical, financial, psychosocial) to the Director of Admissions and/or his/her designee prior to admission.
- Coordinates admission activities and comprehensive documentation with respective departments in accordance with state and federal regulations.
- Acts as a liaison between the facility and medical providers such as hospitals, home care agencies, etc.
- Utilizes appropriate sales/marketing techniques, (public speaking, community workshops, cold calling, facility events, media coverage, etc.), to plan, develop and implement facility marketing plan.
- Assures detailed communication of all necessary pre-admission information in a timely and professional manner.
- Assists in maintaining budgeted occupancy and census mix.
- Communicates daily with the admission department to determine bed availability and review referral data.
- Provides ongoing assistance to potential patients and their families in negotiating a successful placement.
- Assure detailed completion of all admission, discharge and transfer records manually as well as computer generated. Has knowledge of Admission software and be able to utilize effectively.
Specific Requirements:
- Must be able to read, write, speak and understand the English language; a second language is a plus.
- Must possess the ability to make independent decisions when circumstances warrant such actions.
- Must possess the ability to deal tactfully with personnel, patients, family members, visitors, government agencies, and the general public.
- Must possess leadership qualities, the willingness to work harmoniously with professional and non-professional personnel and be a team player.
- Must have patience, tact, enthusiasm and a cheerful disposition.
- Must be able to relate information concerning a patient's condition/situation to the Director of Admissions and/or his/her designee.
- Must maintain professional standards of conduct, appearance and represent the facility and Athena Health Care Systems in a courteous professional manner at all times.
