Job Description:
Cedarbrook Senior Care & Rehab is seeking a Full-time Clinical Admissions Specialist (Nursing Home Admission Liaison) for its Allentown Location.Responsibilities:
  • Evaluate Admissions Referrals for placement in the Facility with a focus on expediently converting appropriate referrals to admissions.
  • Coordinate the Admissions process between referral source, resident, and resident representative/resident family while collaborating interdepartmentally to ensure Cedarbrook Admissions processes and procedures occur.
  • Update relevant electronic systems with outcomes.
  • May be required to occasionally travel to local referral source to evaluate a potential admission.
  • Comply with relevant nursing home regulatory requirements.
  • Other duties as assigned.
  • Shift is 10:00 am - 6:00 pm Monday-Friday with flexibility for evening or weekend coverage as defined by Cedarbrook Administration.
Qualifications:
  • Highly energetic individual with at least 3 years of experience in a skilled nursing facility or equivalent healthcare setting with a focus on positive customer service outcomes.
  • Licensed practical nurse with demonstrated ability to understand and review clinical charts.
  • Proficiency in electronic medical records systems.
  • Proficiency in Microsoft Office products.
  • Preferred to also have had at least 2 years of successful Admissions or Case Management experience.
  • Highly Preferred to also have had at least 5 years of skilled nursing facility experience in either Admissions, Business Development, or applicable Clinical Experience.
  • Demonstrated successful performance in current position.
  • Or any equivalent combination of acceptable training and experience which has provided the knowledge, skills and abilities cited above.
