Job Description:

A health services network in New York City is currently seeking a licensed Registered Nurse (RN) to join their team as a Clinical Assessment Manager. In this role, the Clinical Assessment Manager (RN) will ensure effective and efficient completion of tasks associated with the clinical assessment process.

About the Opportunity:

  • Schedule: Monday to Friday
  • Hours: 8:30am to 5:30pm
  • Patient Population: Geriatric
  • Locations: Manhattan (1 opening), Queens (1 opening), Westchester (2 openings), Wallerstein (1 opening), Flushing (1 opening)

Responsibilities:

  • Work closely with the Intake & Enrollment and Care Delivery teams regarding in-home or virtual assessment appointments with prospective or enrolled participants (utilizing the Uniform Assessment System of New York, UAS-NY)
  • Answer questions about the PACE program and emphasizes the unique benefits of the PACE program
  • Conduct in-home assessments for PACE participants utilizing the UAS-NY to determine initial or continued plan eligibility by the requirement need of Community Based Long Term Care (CBLTC) Services for greater than 120 days, and care needs
  • Promote and educate potential referral sources about the PACE program as needed to encourage participation in the program through presentations, lectures, meetings, or other support
  • Educate potential/new PACE participants on plan benefits, services, and care delivery model
  • Perform other tasks, as required

Qualifications:

  • Prior experience in MLTC, LTTHCP, and/or Managed Care program
  • Graduate of an accredited School of Nursing
  • NYS Registered Nurse (RN) license
  • Solid assessment, clinical, and documentation skills
  • Patient oriented
  • Great interpersonal skills
  • Excellent communication skills (written and verba)
  • Strong attention to detail
  • Highly organized
