We are hiring a Clinical Assessment Nurse (UAS) on a Remote(Virtual) basis.
To qualify, candidates must have 1 year of UAS experience and need to sit within the Bronx, Manhattan, or Westchester. Please note, it will be working from home.
Requirements: Active RN in NY (1 year of UAS experience required)
Hours Monday - Friday 8:30a-5pm (Full time only)
Location: Sit within the Bronx, Manhattan, or Westchester
Remote: Meaning Virtual (Work from Home)
Pay: $48-52/hr
Job Details:
- RN Conducts assessments for prospective members and re-assessments of current members as well as identify enrollment and or eligibility by utilizing clinical expertise and critical thinking skills. As needed, conducts assessments in settings other than home e.g. skilled nursing facilities.
- RN conducts In-person Care Management visits in support of person-centered service planning.
- RN Completes assessment of members needs due to changing conditions.
- Reviews previous nursing assessments/medical notes to determine necessary adjustments/updates in Care Plan and/or to assist with nursing home placement eligibility.
- Coordination with home care agencies, vendors, pharmacies, medical providers and CMT regarding needs identified.
Qualifications
- Must be a self-assured professional with a current New York State Registered Nurse License required.
- 1 years of UAS experience required.
- MS Office required
