- Assists healthcare providers in home visits when needed.
- Assists with coordination of durable medical equipment, medical supplies, and home health services.
- Provides administrative support to the interdisciplinary team by scheduling appointments ,keeping track of referrals, and other services ordered.
- Coordinates access to prescribed medications.
- Take appropriate action to maintain a safe environment.
- Assists healthcare providers with examination as required and assists other staff members in providing patient care.
- Checks equipment and supplies prior to use for function, inspection and expiration date; reports malfunction of medical equipment and safety issues to supervisor.
- Complies with Infection Control and OSHA Regulations (biomedical wastes, handling, and disposal).
- Performs venipuncture and obtains specimens in appropriate containers and labels correctly.
- Performs supervised procedures such as but not limited to EKG, vaccines and medications via IM/SC/ID/Oral and assists in procedures/treatments as requested by the provider.
- Check medical records and follow up obtaining missing results prior to the patient's appointment.
- Responsible for performing telephonic or face-to-face history and program needs assessments using a tool with predefined questions for the identification, evaluation, coordination and management of patient's needs.
- Using tools and a pre-defined identification process, identifies members with potential clinical health care needs (including, but not limited to, potential for high risk complications) and coordinates those member's cases with the clinical healthcare management and interdisciplinary team in order to provide care coordination support.
- Manages non-clinical needs of members with chronic illnesses, comorbidities, and/or disabilities, to insure cost effective and efficient utilization of services. Build care relationships among family and team; support the primary care-giving role of the family.
- Promotes teams and actively participates in daily huddles.
- Care management coordination of non-clinical services such as transportation, follow up on referrals, etc.
- Makes follow up communication to patients/families on non-clinical matters such as confirmation of delivery of equipment, no show appointments, etc.
- Serve as a contact point, advocate and informational resource for patient/family.
- Help educate patients on using medical services appropriately; improve patient/family healthcare literacy - especially for at risk patients.
- Culturally effective capabilities demonstrating a sensitivity and responsiveness to varying cultural characteristics and beliefs. Knowledgeable regarding data collection and interpretation, use of spreadsheets.
- Maintains knowledge of and compliance with current Medicare/Medicaid, state/federal rules, and regulations for professional services.
- Complies with all Health Insurance Portability and Accountability Act (HIPAA) requirements in accordance with federal, state, and organizational policies.
- Assumes responsibility for personal growth. Develops, maintains, and upgrades professional knowledge and practice skills through attendance at seminars, conferences and participation in continuing education and in-service classes.

QualificationsRequired Education
- High School Graduate or equivalent
Required Experience
- 1 year in a clinical setting, previous hospital experience and/or physician office experience
Required Licenses and Certifications
- Medical Assistant Certificate
- Phlebotomist
- CPR
