Description

Starting pay $30.00 per hour

  • Triage clinical calls, pages nurses, transfers calls, takes and distributes clinical telephone messages.
  • Orders medical supplies and maintains inventory of supplies, as directed by Clinical Director.
  • Assumes backup responsibilities for CHHA Scheduler in their absence.
  • Assumes backup responsibilities for field LPN.
  • Maintains and updates case management sheet weekly.
  • Tracks Face to Face visits and communicates with physician and nurse practitioners regarding due date.
  • IDT preparation, including making COTI's, scribing during meeting.
  • Sends daily fax communication to Medical Director regarding new admits.
  • Retrieve information regarding deaths and schedule DME Pickups.
  • Assumes backup responsibilities for Referral Services.
  • Keeps and tracks accurate DME records
  • Provides feedback to referral sources on referrals as indicated.
  • Responds to telephone inquiries regarding available services and eligibility for same.
  • Utilizes SFAX to send and track all orders.
  • Provides coverage at the main office, hospice house, and Pike office as needed.
  • Participates in all required in service education programs and meetings.
  • Demonstrates a commitment to own work schedule by adhering to attendance standards. Provides proper notification in advance for absence and tardiness.
  • Adheres to employment policies in accordance with the Employee Handbook
  • Demonstrates compliance with the dress code policy by appearing well groomed, wearing ID badge while on duty and maintaining a professional appearance.
  • Performs other duties, as assigned.
Position Requirements

Required Skills/Abilities:

  • Knowledge of hospice services as they relate to the assignment of Certified Home Health Aides (CHHAs)
  • Knowledge of administrative office procedures, including the completion of forms, calendar administration and file maintenance and recordkeeping
  • Self-starter who can work independently and make sound decisions under the direction of the Clinical Director.
  • Excellent interpersonal skills for interacting with Aides, nurses, employees, and patients and families via telephone and in person (reception duties)
  • Ability to comprehend the spoken and written English language, to communicate with clarity both verbally and in writing.
  • Well organized to manage workflow, documentation, file maintenance and recordkeeping
  • Proficiency in working with Microsoft Word, Excel, and Outlook, and electronic scheduling and medical record software.

Education and Experience:

Required Education: Graduation from an accredited school of Practical Nursing; licensed to practice as a Practical Nurse in the State of New Jersey.

Preferred Education: Some college or business school.

Required Experience: Minimum one year adult or pediatric medical/surgical clinical experience.

Preferred Experience: Prior home care or hospice experience preferred.
