Job Description:

Jacksonville, FL 32205 The Clinical Care Coordinator is responsible for effective and efficient delivery of non-skilled services. Responsibilities include ensuring qualified employees provide care in accordance with applicable laws and regulations, standards of care, and policies and procedures. Primary Responsibilities:

  • Manage and supervise a team of home care aides, CNAs, companions, sitters, and in-office staff to provide a high level of customer service and the highest quality of care.
  • Work with referral sources, patients, and caregivers to plan and direct non-skilled services to patients in their homes.
  • Perform home visits to prepare the written service agreement and supervisory visits
  • Facilitates initial orientation, competency testing, and in-service training for new and existing personnel and ensures that patient and personnel safety, emergency preparedness, and infection control practices are included in orientation and continuing education and monitors compliance
  • Maintains current knowledge of home care regulations and standards
  • In conjunction with the Administrator, resolves patient complaints, grievances, problematic patient/family issues, and unusual aide requests using effective communication
  • Maintains confidentiality of patient, employee, and agency operations
  • Reviews and ensures all incident reports for accuracy, completeness, and risk level, and follow-up needs
  • Implements processes to monitor and evaluate trends and coordinates/prepares trend reports on safety, risk management, adverse events, infection control, outcomes, patient, referral source and personnel satisfaction and grievances, and record documentation of service provided
  • Reviews aides' documentation to assure that visits are made per the written service agreement and performs audits as necessary.
  • Ensures the development and implementation of policies and procedures and coordinates the annual review of policy and procedure
  • Participate in organizational committees as identified that enhance the working relationships between departments within Alivia Care, Inc.
  • At all times, represent Alivia Care, Inc. professionally through care and/or services provided to all clients and patients
  • Comply with Alivia Care, Inc. and Alivia Care Solutions policies, procedures, and standard practices
  • Use resources in a fiscally responsible manner
  • Promote Alivia Care, Inc. and Alivia Home Health through participation in community and professional organizations
  • After-hours work including rotating schedule of nights, weekends, and holidays. Ability to be contacted during all hours of patient service.
  • Work within scope of licensure and regulations. May not provide skilled care under a non-skilled license.
  • Attend regular staff meetings as scheduled.
  • Other duties as assigned.
Qualifications and Requirements:
  • Current Florida licensure as a Registered Nurse
  • Current driver's license and dependable insured automobile
  • Graduate of an accredited School of Nursing (Bachelor of Science in Nursing (BSN) preferred.)
  • Minimum three years clinical experience within the last five years, preferably in home health care and at least one year of supervisory experience
  • Current CPR certification that meets state requirements
  • Level II ACHA background screening
  • Possess knowledge of home health care regulations and procedures
  • Possess effective communication skills and ability to lead a team of personnel with a variety of backgrounds and educational experiences
  • Possess good time management, problem-solving, work ethic skills

  • Computer proficiency with Microsoft Office including Word, Excel, Outlook, and PowerPoint
