Job Description:
Center for Human Development (CHD) is currently looking for a Clinical Care Manager to join our team at our West Springfield, MA office. The Innovative Care Partners (ICP) division provides care coordination and care management for member of our community. Our goal is to apply innovative care coordination, information technology, and human expertise to improve health outcomes for clients enrolled in the MassHealth Behavioral Health Community Partners initiative. This position is responsible for supervising, and providing leadership to a team of Care Coordinators to assure the timely facilitation and coordination of assigned enrollee care.Your role as a Clinical Care Manager:
  • As a clinical resource person at the site, serves as role model and mentors others in the decision-making process within the context of the Community Partner model of care;
  • Closely observes, assesses and evaluates skills and team performance by actively participating in case conferences; conducts team and 1:1 meetings with staff as indicated;
  • Provides direct supervision, evaluation and training of Care Coordinators
  • Ensures that Care Coordinators document all Enrollee-related activities in the health record on a timely basis and in compliance with established standards
  • Supports staff, and ensures that staff receive training as needed, in evidence-based treatment models such as motivational interviewing and trauma informed care
  • Liaisons with community agencies, behavioral health providers, substance use disorder providers, primary care providers, ancillary services, community advisory board, family members and other stakeholders to promote strong relationships and opportunities for Enrollees
  • Facilitates transition plans and the implementation of same.
Required:
  • Master's Degree in social work, psychology, sociology, OT or LADAC
  • Must have a vehicle available for work purposes
  • Position is subject to background record check including CORI, motor vehicle record check, Office of the Inspector General (OIG) List of Excluded Individuals and Entities
SUCCESS FACTORS:Our ideal candidate with have previous experience in the delivery of behavioral health services, long term services and supports, care coordination/care management techniques and practice. Displayed strength in behavioral health and medical assessment, decision-making and developing plans of care to support colleagues, individuals and teams involved with complex care management/care coordination issues.This is a Full time opportunity. Salary range Salary: $72,000 - $74,000.00 per year, negotiable dependent on licensure and experience.
