Overview:CASES seeks aClinical Case Coordinatortojoinits Intensive Services Unit within Pretrial Services. This initiative will implement innovative approaches to working with individuals who have been placedin theSupervised ReleaseProgramand experience more extensive needs and challenges that mayimpactdaily functioning andoverallprogram engagement. Theideal candidate for this position will work well as part of a coordinated clinical team and serve participants in direct collaboration with a Peer Specialist as part of a coordinated approach to care.The focus group of program participants are managingchallengesin the areas of mental health, substance use,employmentand medical health.The team promotes proactive, sustained engagement in the community as a catalyst to individual transformation by bridging participants to community partners and resources eager to support our participants in realizing their greatest potential. TheClinical Case Coordinatorwill be an active partner in providing direct services to participantsprimarily in the community withadditionalservices being provided at other CASES program locations.Location: 4 West 125th Street New York NY 10027.Salary: 65K AnnuallyHours: Full-Time Monday - Friday 9AM-5PMWhat you will be doing:Provide community-baseddirect servicesto alimited, specializedcaseload of pretrial participantsthat may include ahigher recidivism risk,participants with mandated treatment requirements,and thoseindividualswithhigher profiledcases.Rapidlyat the point of arraignmentconductIntake Assessmentstoidentifyimmediate critical needs that must be addressed tofurther sustain program involvement andenrollment.Participate in regularly occurringteam meetings and case conferences to ensure seamlesscoordination among staff, wide dissemination of participant information as well as rapid responses to urgentneeds.Conduct review of legal and court screening documentation tofacilitateadmission and ongoing needs assessment of criminal history, psychosocial, developmental, and behavioral health needs, supervision risks toidentifyengagement approach, focus for behavioral change, and needed supports to reduce re-arrest, and promotesuccess in the community following programcompletion.Serve as a designated staff person forhighriskand high needscases, coordinating andparticipatingin case reviews and case conferencesto ensureacoordinated,collaborative approach for complexcases.Facilitate effective collaboration with staff delivering court services to ensure theefficient enrollment of new participants into the program while gathering the necessary basic information for ongoing serviceprovision.Link participants to emergency, housing, education, vocational, employment, and treatment services based on their preferences and needs, providenavigationto community services, andestablisheffective collaboration and positive relationships with communityproviders.Utilize problem solvingandmotivational interviewing to help motivate and engageparticipantsinthechange process,ensuring the supervision and case management approach is culturally competent,gender-responsive,trauma-informed, andadheres to evidence-basedrisk-need-responsivityprinciples.Provide culturally competent and anti-racist services 1)in accordance withCASES' policies and the delivery ofqualityservices to address the needs of youth and young adults, adults, and individuals withbehavioral health needs and 2) that are responsive to the race, ethnicity, sexuality, gender, and criminal legal history of programparticipants.Ensuretimely, complete, andaccuratedocumentation in the case record database including assessments, supervisionschedulesand progress notesin accordance withconfidentialityregulationsParticipate in clinical supervision, case review, staff meetings, and in-service trainings to develop clinical, motivational interviewing, trauma-informed, supervision,case management, and self-carecompetencyStrive to meet monthly supervision, casecoordination, and outcomesgoals.Any other duties as required by theTeam Leader, Intensive Services Unit to support the overall functioning of theprogram.What we are looking for:Master's degree in social, Mental Health Counseling, Psychologyor other relevant clinical discipline.Active licensure in a clinical discipline in New York State,preferred.One to two years of experienceprovidingclinical services to individuals tofamilies, orgroups managing challenges in the areas of Behavioral Healthandthe Criminal Legal System.Experienceprovidingdirect clinical services in a field-based setting stronglypreferred.Strong interpersonal skills and emotional intelligence.Ability to work effectively within a diverse teamenvironment.For more information about CASES benefits please visit:Careers - CASESCompensation details:65000-65000 Yearly SalaryPI48fcf9a40d01-35216-#######9
