Must be willing to travel between our two (2) facilities located in Carlsbad.Salary Ranges:Clinical Case Manager I - $65,500-$68,000.Clinical Case Manager II - $68,000-$78,000.Benefits:Medical, Dental, and Vision plans through Anthem, with required payroll deductions from the employee. Other duties may be assigned.Proactively develop person-centered care plans, including prioritized problem lists, SNAP, client-reported goals, measurable and achievable objectives, and interventions. Identify and prioritize individual needs and problems.Be responsible for the client's aftercare plan and make decisions based on the best interest of the client.Lead and provide group therapy sessions utilizing Evidence Based Practices.Case load of up to 12 clients, with minimum of weekly aftercare planning sessions.Coordinate and manage client transfers to higher or lower levels of care and discharges against clinical advice.Communicate with aftercare programs and business development, facilitating aftercare coordination and coordination of care between programs.Initiate and develop comprehensive transition and discharge plans and summaries for assigned caseload.Prepare clinical documentation accurately and within agency time frames.Intervene during crisis intervention situations.Supervise and participate in the agency's Quality Management system, Orientation, staff meetings, and in-service training.Provide accurate assessment and referral when appropriate.Provide appropriate treatment interventions relative to the specific needs of each client.Adhere to code of ethics, legal aspects of clinical practice, professional standards, duty to warn, abuse and neglect reporting policies and procedures, and exercise professionalism at all times.Qualifications and Skills Required:To perform this job successfully, an individual must be able to perform each essential job function assigned satisfactorily.
