POSITION SUMMARY:The Clinical Charge Nurse, Hospital at Home demonstrates leadership through the assessment, planning, implementation and evaluation of patient care according to the standards of professional nursing practice and is a resource for program leadership as appropriate for precepting, scheduling, team meetings, and clinical implementations as appropriate.PRIMARY ACCOUNTABILITIES:Demonstrates appropriate delegation and supervision of patient care activities within the program.Demonstrates effective communication with other members of the healthcare team and promotes effective team functioning.Helps to monitor the effectiveness of existing processes and policies towards improving quality outcomes; actively participates on quality projects.Recognizes and resolves safety issues and potential unsafe situations within approved policies, procedures and protocol.Actively participates in the development and attainment of department team goals.Demonstrates effective critical thinking, prudent judgment and direction to staff in nursing assessment, diagnosis, planning, interventions and evaluation of outcomes, and adjusts plan of care as needed.Facilitates and leads effective interdisciplinary care team meetings as well as serves in a coaching, mentoring, or instructive role to nurse interns, students and peers, etc.Effectively leads shift and post-event huddles when appropriateParticipates and provides feedback to manager for associate performance management and hiring interviews.Demonstrates effective conflict resolution skills, demonstrates dependability, reliability, and flexibility.Creates and sustains a healthy work environment positively contributing to staff retention, orientation, and onboarding.Serves as a resource to address points of escalation with patients, families and staff.**MINIMUM QUALIFICATIONSEducation: o Graduate of an approved school of professional nursing. o Associate Degree in Nursing or Nursing Diploma required.Work Experience: At least two (2) years of acute care, medical-surgical or home health clinical experience preferred.Licensure: o Current Florida RN licensure or endorsement. o Must have and maintain current driver's license, auto insurance and reliable transportation.Certification: Must hold current AHA BLS Healthcare Provider Completion Card prior to start date and maintain.Knowledge/Skills/Abilities: o Must successfully complete eligible Level II background screening. o Clinical competency, including IV, nursing assessment, medication administration and other clinical skills as needed. o Proficiency with MS Office, Outlook, Electronic Medical Record (EMR) and other applications and systems related to role. o Ability to manage time, prioritize multiple conflicting tasks, and work autonomously. o Demonstrates teamwork, critical thinking, and sound judgment in decision-making. o Excellent written, oral and auditory communication skills with focus on customer service. o Adaptable, flexible with workload, work schedule/schedule, including on-call rotation. o Financial acumen and familiarity with regulatory documentation.PREFERRED QUALIFICATIONSEducation: BSNWork Experience: Progressive Care/Emergency Medicine experience desired.PHYSICAL REQUIREMENTSPhysically agile; may require walking, standing running, bending, stooping, kneeling, climbing or crouching frequently.Medium work that may include lifting or moving objects up to 50 pounds of force occasionally, with or without assistance.May require moving self in different positions to accomplish tasks in various environments including tight and confined spaces, including reaching out and above shoulders or over head.May lift, turn, pull, push, or ambulate people and/or equipment with or without assistance up to 90% of the assigned shift.May be exposed to inside and outside environments with varied temperatures, air quality, lighting and/or low to loud noise.May require working with biohazards such as blood/body fluids and airborne particles following OSHA and CDC Guidelines.Communicating with others to exchange information.Visual acuity and hand-eye coordination to perform tasks.Workspace may vary from open to confined; on site or remote.May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.Job: *Registered NurseOrganization: *Holmes Regional Medical CenterTitle: Clinical Charge Nurse - Hospital at HomeLocation: Florida - Brevard County-MelbourneRequisition ID: 075438
