Duties & Responsibilities: •Assist in the hiring, orientation, supervision, scheduling and evaluation of clinical staff.•Monitor and facilitate the flow of care plans and ensure their accuracy.•Teach and supervise written documentation, charting and tracking of care plans.•Assist the Director in taking referrals, performing initial evaluations and distributing referrals to staff.•Carry small caseload of patients.•Plan staff in-service education programs and facilitate and track staff attendance at educational conferences.•Participate in the on-call system.•Assist and support all process and quality improvement projects for the Home Health department (QAPI).•Employ financial acumen in budgeting.•Replenish the facility's medical supplies and keep track of expenditures.•Attend and contribute to departmental meetings.•Implement procedures across the department.•Assist in the development and marketing of new programs/services as needed.•Handle other responsibilities as assigned by the Director.QualificationsMinimum Basic Knowledge:Minimum of 1 year of work experience on a medical surgical unit and 3 years of experience in home health care required. Experience & Qualifications:Registered nurse licensed in the Commonwealth of Massachusetts. BSN degree required; MSN degree preferred. Independent Action:Direct client care under physician's orders. Supervisory Responsibility: Assist the Chief of Home Health in the supervision of all nursing staff. Define Access Level to PHI: Level 4: Authorized to access full health information. UCC position and job responsibilities involve the provision of patient care and working as a clinical team member. Staff in this category level, although allowed full access, should only access the necessary information for each respective treatment encounter/circumstance.
