Job Type Full-time Description Sign on Bonus of $10,000Summary/Objective: As a CHJC employee, the Foster Care Clinical Coordinator (Clinical Coordinator) is committed to creating opportunities to enhance lives one relationship at a time. Under the leadership of the Foster Care Program Manager, the Clinical Coordinator provides direct oversight of the provision of mental/behavioral health services and treatment planning for foster care youth. To assess the need for clinical services within an outpatient clinical setting, the Clinical Coordinator works with the treatment team for each youth entering the Foster Care Program. When clinically necessary, the Clinical Coordinator works to ensure services are available and provided to foster care and community-based youth. Through foster parent/guardian and staff trainings/support groups, the Clinical Coordinator is responsible to strengthen competencies and provide education on various topics. Essential Supervisory/Administrative Functions:

  • Provides direct individual/group/clinical supervision and/or oversight of Foster Care clinical and nursing staff, as assigned
  • Monitors staff schedules, job performance, and time and attendance; approves time off and completes required performance evaluations for assigned staff
  • Mentors and monitors ongoing professional development training for staff, completes staff development plans, and ensures ongoing/necessary trainings are provided to all staff
  • Collaborates with HR team on the hiring process for new staff (reviewing qualifications, facilitating interviews and reference checks, etc.)
  • Oversees all necessary data trackers
  • Works with other agency team members in on-going agency-level quality improvement activities
  • Ensures compliance with CHJC's overall policies and procedures and the regulations set forth by New York State Office of Children and Family Services (OCFS) and Office of Mental Health (OMH)
  • As needed, ensures CHJC policies/procedures are updated to reflect any necessary changes and ensures training/awareness to all program staff of any change
  • Participates in weekly supervision with the Program Manager
  • Participate in the on-call rotation and responsibilities as assigned
  • Essential Program Functions:
  • Through on-going collaboration with Foster Care leadership, mentors staff and provides clinical guidance regarding treatment for foster care youth
  • Serves as a liaison between Foster Care and Clinic Programs
  • Works collaboratively with the Clinic Practice Manager to ensure treatment needs are met for foster care youth
  • Serves as a clinical consultant and guides treatment for foster care youth
  • Responsible for collaborating with Foster Care Program staff to assist in planning and optimizing behavioral health treatments and supports
  • In collaboration with clinical and administrative staff, ensures all youth within the Foster Care Program receive all required assessments, safety plans, Individual Crisis Support Plan, treatment plan, etc.
  • In collaboration with program staff, completes mandated documentation for 29-i
  • In collaboration with program staff, monitors youth's progress toward achievement of treatment goals and recommends additional services as needed
  • Provide behavioral health and psychotherapy services to youth in the foster care program and community
  • In collaboration with program staff and applicable treatment teams, leads timely crisis response efforts for program youth
  • Participate in regularly scheduled family meetings, treatment team meetings, and service planning meetings
  • To prepare for discharge, collaborate and participate in discharge planning meetings
  • Completes Initial Assessments for youth entering foster care referred, and reviews treatment plans
  • Reviews all clinical/service provider documentation as required
  • Essential Clinical Functions:
  • In collaboration with Clinic leadership, participates in utilization and clinical reviews
  • As required, supervises assigned interns
  • Special Requirements: Work Environment: Office environment will require occasional travel to conferences and/or meetings and moderate travel within the community to include off-site/in-home visits. May have contact with service recipients that are agitated and/or are in crisis. Some risk involved working with service recipients with mental illness. Works in an office setting with a controlled temperature environment. Occasional exposure to inclement weather conditions may occur depending upon assignments. Equipment: This position is required to maintain a working knowledge of related office equipment including personal computers and printers, audio-visual equipment, telephone systems, copiers, fax machines, etc. Physical Demands: Must be able to sit, stand, walk, lift, carry, push/pull, climb, bend, and stoop. Must be able to perform fine motor skills, read, and type. Must be able to sit for long periods of time. Position Type/Expected Hours of Work: Full-time, traditional hours required along with occasionally some non-traditional hours. This position is required to work on-site, and offsite/in home visits. On-call rotation. Travel: Travel is required throughout the service area; specifically, between CHJC's Watertown and Canton Offices. Required Education and Experience: Requirements: Current licensure in New York State as a Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), Licensed Mental Health Counselor (LMHC), Licensed Creative Arts Therapist (LCAT), or Licensed Marriage and Family Therapist (LMFT). Preferred experience working with youth in Foster Care and families involved in Foster Care. Strong written and oral communication skills, excellent time management skills. Requirements Skills/Abilities/Knowledge: Strong organizational abilities and office skills are essential for this position. This person must also have excellent people skills and be a team player. Ability to work with diverse populations. Must demonstrate effective and excellent oral and written communication skills and the ability to work cooperatively with others. Must be detail-oriented and possess excellent time-management skills. Requires self-direction, tact, diplomacy, and the ability to be clear, courteous, and professional. Able to react to change productively and handle other duties as assigned. Additional Eligibility Qualifications: Must maintain privacy of confidential information. Successful completion of routine pre-employment requirements as mandated by the New York State Department of Health, Office of Mental Health and Office of Children and Family Services along with ongoing clearances and exclusion checks. These requirements are to include employment health requirements, reference checks, criminal background checks/fingerprinting, State Central Registry clearance and your eligibility to work in the United States. A valid NYS driver's license required. Corporate Compliance, HIPAA Privacy Security, and Workplace Harassment and Discrimination Training and Nonviolent Crisis Intervention, First Aid and CPR will be required following date of hire.
