Supportive housing - affordable housing paired with services designed to help people maintain their homes for the long-term - is widely recognized as a proven and cost-effective solution to chronic homelessness.But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.Reporting to the Assistant Director, Living in Place, the Clinical Coordinator, Living in Place, provides clinical support to Queens based, Aging Services programs at Breaking Ground using a Critical Time Intervention (CTI)-informed model; providing time-limited services focused on housing retention, resource development, and community integration intended to support older adults with aging safely in the community, and provides information and referral services; benefits application and navigation assistance; resource development support; and community integration programming. The case management for Aging Services is typically a 3-6 month engagement and on a referral basis.The Clinical Coordinator maintains a caseload of 30-40 clients and serves as a member of the Tenant 360 management team.This position has a 35-hour work week. This is a fully in-person position that will divide time across the Breaking Ground building portfolio. The position may require occasional evening or weekend hours to support tenant events.ESSENTIAL DUTIES AND RESPONSIBILITIES:* Supervise Aging Services Specialists and Consumer Advocates* Liaise with the Occupational Therapist* Maintain a caseload of 30-40 clients* Facilitate and provide resources for ongoing education and awareness among staff about aging-related issues, advancements in geriatric care and best practices* Develop and oversee individual and group programming and service delivery workflows that support overall programmatic goals* Assist with standardization of data collection, data analysis, internal reporting for continuous quality improvement, and external reporting for funders is conducted in a timely manner* Travel to buildings to support program delivery and event logistics, including evening and weekend hours when necessary* Perform other duties as assignedMINIMUM QUALIFICATIONS:* NYS Licensed Masters Social Worker (LMSW) required. Must obtain LMSW within first six months of employment* Minimum 3 years of experience with related populations; previous supervisory and administrative experience preferred* Thorough clinical understanding of relevant service delivery concepts and structures including Housing First, client centered motivational interviewing, Harm Reduction models, behavioral and medical health systems, and the ability to access and negotiate the full range of services for recipients* Experience with crisis intervention, including, risk assessments and incident management* Ability to work successfully with a wide range of internal and external stakeholders* Demonstrated success in working in a fast-paced environment with multiple priorities* Ability to delegate and motivate staff to achieve deliverables* Ability to communicate (verbally and written) with diverse populations and stakeholders* Proficiency with Microsoft Office Suite programs including Word, Excel, Outlook, and Microsoft Teams; familiarity with Access-based databases and the ability to learn new programs are required* Valid NY or NJ driver's license with a good driving record, as defined by Breaking Ground is preferred* Bilingual preferredEOE/M/F/Vet/DisabledOther details* Pay Type Salary* Min Hiring Rate $64,000.00* Max Hiring Rate $68,000.00Apply Now* 3-24 27th Ave, Astoria, NY 11102, USA
