Overview Directs and coordinates management/staff in the delivery of patient care services for the Licensed Home Care Services Agency (LHCSA). Responsible for the delivery of quality cost effective home care services to patients/families in compliance with LHCSA procedures, standards, policies and regulatory requirements. Works under general supervision. Responsibilities Directs and supervises administrative and clinical management in delivery of effective patient care services ( e.g. admissions, discharges, provision of services, etc.) in compliance with Agency/regulatory requirements governing Homecare. Leads and manage s a team of Nurses and caregivers, promoting a culture of compassion, professionalism, and excellence. Oversees all patient care plans, ensuring they meet client needs. Directs and supervises administrative staff, ensuring efficient clinical scheduling and overall productivity. Reviews all clinical documentation for timely submission and signing of MD Orders. Collaborates with clinical specialties and other clinical management staff in the implementation of patient care. Analyzes and resolves complex clinical and financial case management problems in collaboration with leadership in Operations, Compliance, Legal and Finance Departments. Participates in the development and implementation of the Quality Assurance Program. C onducts regular staff and development meetings to enhance service delivery and patient care skills. In conjunction with leadership, ensures that staff maintain expertise in the field and are up to date on clinical issues through orientation, in-service education and continuing education. Conducts in person patient visits and field supervision as required for all clinical positions to ensure compliance. Participates in the clinical on-call rotation. Collaborates with personal care leadership to implement, monitor and maintain cost-effective patient services. Develops, coordinates, evaluates and implements communication/workflow procedures in collaboration with senior management to maximize optimal clinical/operational service delivery. Identify solutions and resources needed for maximum performance and efficiency. Maintain and update all assessment requirements in collaboration with the IT department. Ensures that the highest quality of patient care is provided 24-hours, 7-days a week. Oversees coordination/management of management/staff schedules to meet round the clock operation of clinical services. Fosters a positive relationship between caretakers and patients resulting in provision of quality services. Performs all duties inherent in a director level position, including, hiring, training, supervising, evaluating, recommending salary actions and when necessary, terminating assigned staff in consul t ation with Executive Director, LTC and in accordance with LHCSA policies and procedures. Participates in special projects and performs other duties as assigned. Qualifications Education: Bachelor's degree in nursing Required. Master's degree in nursing or other health related field Preferred. Experience: Minimum of five years of progressively responsible clinical management experience in a health care setting Required. Strong knowledge and understanding of the regulatory requirements governing home care in LHCSA. Licenses, Certifications: License and current registration to practice as a Registered Nurse in New York State Required. Physical Requirements: Consistent with that necessary to work in a variety of office settings. Position requires prolonged p eriods of s tanding, reaching, and walking throughout the workday. Position will be required to stoop, bend, lift, and carry items weighing up to 25 pounds for routine performance of essential functions.
