The clinical educator assists in the initial and ongoing education process for clinical staff within the Certified Home Health Agency (CHHA) and Licensed Home Care Services Agency (LHCSA). Particularly in skills review, in utilizing the electronic medical record appropriately and in the adherence to agency policies and procedures. Provides direct care as part of maintaining individual skills, and for the purpose of mentoring others. Performance Responsibilities and Standards: 1.Ensure quality and safe delivery of home health services.
- Instruct new staff in agency policy and procedure
- Verify appropriate technique in demonstration of new skills in both the home and simulated setting
- Provide support and ongoing education for existing staff with identified performance deficits
- Provides direct patient care as appropriate to meet agency needs or as demonstration to staff.
2. Provide individualized guidance to clinical staff.
- Thru joint visits, direct observation, chart review, and other methods as needed to track development of new staff or existing staff
- Ensure compliance of new staff or existing staff under with established standards of documentation to provide evidence of skilled, medically necessary intervention.
- Employ the principles of adult learning to convey new or changed information or to elicit performance improvement with staff
- Instruct staff in principles of time and utilization management to deliver efficient, effective care
- Mentor new staff in the role and responsibilities of a case manager
- Provides input to aid in the completion of the new hire evaluations for clinical staff at 90 days and advises to extend probations if necessary

4. Participate in personal growth and development.
Participate in educational in-services and meetings. Maintain and update clinical knowledge on an ongoing basis. Performs all other duties as assigned Requirements:
Current New York State Registered Nurse professional license. Degree from an accredited school of nursing or college. One-year experience as a public health nurse in a home care agency. Current and valid driver's license and auto insurance and a vehicle which can be used for work. Self-directed and organized with the ability to work independently. Good oral and written communication skills. Bachelor's degree or higher in nursing or related field preferred Physical Requirements: 1. Speaking/visual/hearing ability sufficient to comprehend and express written/verbal communication. 2. Ability to perform tasks involving physical activity including: Pushing/pulling - typical weight 50 pounds; maximum weight 150 pounds Lifting and carrying- typical weight 25 pounds; maximum weight 40 pounds Patient transfers, extensive bending, standing, kneeling, walking in various weather conditions, e.g. snow, rain, sleet and going up and down stairs. 3. Ability to drive on a daily basis in all weather conditions. 4. Bloodborne Pathogens Exposure Category I - Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.
