SUMMARYClinical Intake Coordinator carries out specific substance use, and abuse prevention strategies identified in the treatment plan for the child(ren) or adolescent(s) assigned to their care. This Clinical Coordinator will collaborate with the Program Director to implement the service plan developed- based on the ASAM and additional l assessments. This role may involve the provision of 1:1 services and group services.Clinical Intake Coordinator is responsible for facilitating age-appropriate psychoeducational group sessions inclusive of life skills. The individual will also serve as Clinical intake Coordinator for the youth by managing the execution of the service plans. The Clinical Coordinator will be responsible for making referrals to identified services, maintaining relationships with service partners (including schools), and tracking progress, assuring that the consumer's needs are coordinated, among other programs, agency departments, outside agencies and funding source.
THE FOLLOWI NG ARE THE ESSENTIAL FUNCTIONS OF THIS POSITIONCommunication
- Electronically complete case management services documentation weekly and submit notes monthly per each consumer on caseload via Credible Electronic Records program.
- Accurately complete documents to ensure continuity of service.
- Promotes, creates, allows and assures effective communication among the treatment team members.
- Clearly and effectively communicate pertinent information to responsible parties.
- Advocate for the needs of the consumer to accomplish treatment objectives.
- Communicates pertinent treatment information to the Program Director
- Collaborate with other programs and departments to assure continuity of service for designated consumers.
Documentation
- Completes all progress notes weekly to assure re appropriate billing of services
- Completes all documentation in a timely fashion and in required formals,
including, but not limited to progress notes, incident reports, and encounter forms.
- Documents all therapeutic interactions, unusual incidents, routine activities, etc. as required by policies and procedures.
Planning
- Plans and organizes interventions that support i n the social, emotional and educational development of the consumer.
- Attending interagency meetings to ensure continuity of service.
- Attending mandatory in-service training.
- Attends psychiatric and medication evaluation to assure service delivery.
Monitoring
- In accordance with the principles of best practice, it provides oversight and direction to assigned children.
- Assist the parent, teacher or other responsible adults in their efforts to provide direct supervision of the child(ren).
- Maintain coordination through interagency meetings.
- Assess and reassess the service needs of the designated consumers.
Direct Service
- Provides ongoing support to consumers and other staff as needed.
- Creates an environment which fosters growth and development for all consumers.
- Provides interventions to promote social growth in all consumers.
- Directly implements components of the treatment plans developed in conjunction with the treatment team.
- Provides regular/ routine feedback to team regarding relevance of treatment plan, need for alternative interventions, etc.
Networking
- Represents agency at conferences, training and interagency meetings.
- Helps the consumer keep all appointments (parents, teachers, medical, school, etc.) by coordinating with appropriate personnel.

Education /Experience:- Master's degree: Must be from an accredited college with a major in chemical dependency, psychology, social work, counseling, or nursing (with a clinical specialty in human services) or related field and at least one-year of clinical experience (minimum of 1,820 hours) in a health or human services agency, preferably in a drug and alcohol setting.
Special Skills: Familiar with multiple child-serving systems (e.g., education, juvenile justice, child welfare, mental health, drug and alcohol, health care, and vocational rehabilitation).Youth Engagement: Strong understanding of principles and applications of youth engagement principles.PHYSICAL CAPABILITIES AND WORK REQUIREMENTSAbility to occasionally lift to 20 lbs., up to 10 lbs. frequently, and negligible amounts constantly.Ability to reach, finger, and grasp with hands and fingers.Ability to talk, hear, and speakEmployees are required to have visual acuity to determine accuracy, neatness, and thoroughness of the work assigned, and to make general observations.Flexible hours required with ability to work some nights and weekends.Valid driver's license with ability and willingness to drive within Philadelphia and surrounding counties.WORKING CONDITIONS:Most work is conducted in an office environment and is not substantially exposed to adverse environmental conditions.Trauma-Informed BehaviorAbility to exhibit:
