Alteas is rapidly growing and we'd love to have you join our team with a $5,000 sign on bonus!We are seeking an in-person Clinical Integration Practitioner (CIP) to play a pivotal role in seamlessly integrating new nursing home facilities into the Alteas care network. This position combines leadership, clinical expertise, and operational excellence to drive quality care and provider support.***Alteas has openings for this opportunity in Milwaukee, Kansas City, Chicago and Columbus.***WHAT YOU'LL DO: • Serves as the primary point of contact, introducing and integrating services with facility leadership, nursing and administrative staff• Leads cardiac and pulmonary education sessions, focusing on conditions and clinical enhancement tools and resources • Conducts regular facility rounds, managing both in-person and telehealth visits within growing regions• Oversees provider productivity, address facility-level challenges and collaborate closely with facility staff to ensure efficient operation• Presents and interprets clinical data, creating case studies based on provider input and engagement to improve patient care and outcomes• Supports regional growth by mentoring new providers, ensuring smooth handoff and alignment with facility care standards• Covers facilities for providers on extended PTO • Manages higher volume buildings through in-person rounding• Attends QAPI meetings, represent providers as needed and support ongoing education and quality assuranceWHO YOU ARE: • Current and valid NP licensure in one or more of Alteas' practicing states (IL, IN, KS, MI, MO, OH, or WI) • Access to insured, reliable transportation • 6 months of prior NP experience • 6 months of prior experience in a traveling clinical role • 1+ year of experience in high-acuity care settingApplicants must be licensed in IL, IN, KS, MI, MO, OH or WIPREFERRED QUALIFICATIONS: • Multistate licensure • 2+ years Nursing Home Experience • 2+ years of Cardiology or Pulmonary experience SKILLS NEEDED FOR SUCCESS: • Compassionate Resilience • Integrity and Humility • Innovation • Demonstrate good judgment and problem-solving. • Effective planning, communication and organizational skills WHY YOU'LL LOVE WORKING WITH US:• Sign-on Bonus; $5000 • Competitive Salary and Travel Allowance • Licensure Reimbursement • Continuing Education and Mentoring Programs • 401k • Insurance Benefits (waiting period may apply) o Medical (50% employer paid) o Dental o Vision o FSA or HSA options o Employer Paid Life o Short Term Disability, Long Term Disability, Critical Illness & Accident Coverage Focused on advancing the delivery of patient care by empowering the growth and success of our employees and valued partnerships. www.alteashealth.com
