177124 *Summary*Supports the department Manager and Director in the daily operations of the department; and in mentoring and professional development of staff. Responsible and accountable for translating the mission, strategic goals and department priorities. Supports and maintains a culture of continuous process improvement as related to care coordination and case management. Works collaboratively with the health care team to remove barriers and achieve the desired outcomes for patient/families. Develops informational and educational content for department meetings as required. Supports manager in leadership and development opportunities and provides performance input to the manager for staff yearly appraisals.*Education*Registered Nurse: Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred.ORSpeech-Language Pathologist: Must hold at a minimum a Master's Degree in Speech-Language Pathology from a school accredited by the Council on Academic Accreditation in Audiology and Speech Language Pathology (CAA).OR Physical Therapist: Must be a graduate of an accredited school of physical therapy, with a physical therapy degree as follows: Minimum of a Bachelor's degree, if graduated prior 1/1/2016; Doctorate of Physical Therapy required, if graduated after 1/1/2016; International Candidates must be a graduate of an accredited school and possess a minimum of a Bachelor's degree in Physical Therapy. Doctorate of Physical Therapy degree preferred.OROccupational Therapist: Must be a graduate of an accredited school of Occupational Therapy, with an Occupational Therapy degree as follows: Minimum of a Bachelor's degree if graduated before 1/1/2002; Master's degree required if graduated after 1/1/2002; International Candidates must be a graduate of an accredited school and possess a minimum of a Bachelor's degree in Occupational Therapy. A Master's degree is preferred.*Licensure/Certification*State licensure as Registered Nurse, Physical Therapist, Occupational Therapist or Speech Language Pathologist. Must have good verbal and written communication skills. Must be available to work flexible hours, assuming on-call duties on weekends and other times when deemed necessary. Must be available to respond within a reasonable length of time. Possesses strong critical thinking skills. Ability to lead and direct co-workers in a positive and professional manner. Demonstrates continued professional development. Strong working knowledge of information technology.
