CLINICAL MANAGER - HOME HEALTH SIGN-ON BONUS $5,000.00Position SummaryAre you looking for extensive support in your role as a Clinical Manager? The Clinical Manager is responsible to manage the complex, interdisciplinary care of agency patients with a focus on patient care outcomes. Agency patients include North Hill community Residents and patients living in the broader community. This position oversees all Team Members and all patient care services provided by the agency, whether directly or under arrangement to meet patient care needs. The supervision of agency Team Members includes assigning personnel, assisting in developing personnel qualifications, and developing personnel policies, as well as conducting performance evaluations on an annual basis.Job Specific DutiesReasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.MUST HAVE HOME HEALTH CARE MANAGEMENT EXPERIENCEMUST KNOW STATE and FEDERAL REGULATIONSOversees the agency services to all patients, including staffing needs and schedules, referrals and intake of new patients and delivery of servicesManages the interdisciplinary care of patients through the interdisciplinary team, both employed and under arrangement Team MembersEnsures that patient needs are continually assessed, that care is coordinated, and that plans of care are updated on an ongoing basisProvides supervision to all certified home health care clinical teamCommunicates with referral sources and payer sources, i.e. Tufts Health Care, regarding all aspects of referrals and authorizations for servicesCommunicates and collaborates with all Team Members on an ongoing basis to ensure that all services are timely and that there is ongoing communication across the teamConducts regular interdisciplinary care coordination meetings and case conferences with all interdisciplinary Team MembersFacilitates the interdisciplinary process with the NHHHC team, physicians and other involved professionals through the recognition of the contributions of all disciplines and facilitates interactions and ongoing communication across the interdisciplinary teamResponds to all issues from patient, caregivers and patient representatives regarding care delivered to the patientTracks the completion of all coding and OASIS, and ensures the timely submission of OASISConducts ongoing review of documentation to ensure that all case records are in compliance with Medicare requirements including providing clinician feedback and supervisory visitsCoordinates with the Administrative Coordinator regarding the tracking and receipt of documents required for billingCoordinates with responsible parties regarding revenue cycle managementIn conjunction with the Administrator, Finance Department, oversees and ensures timely response to all ADR requests and all other Medicare/CMS concernsEnsures that all services are delivered according to Medicare regulationsContributes to all phases of the QAPI program, including identification of problem areas, recommendations to address problem areas, data collection, attendance at periodic QAPI meetings as a member of the QAPI committee, and establishment and participation in performance improvement projectsReviews all Home Health Compare, CASPAR, and DEYTA reports on a quarterly basis, and reports all changes and pertinent information to the AdministratorMonitors the documentation of adverse events, falls and infections, review patient records, and aggregates quarterly data for reporting to the clinical team and to the QAPI CommitteeActs as the infection preventionist for the agency through focused activities with the interdisciplinary team to prevent, investigate and manage the spread of infectionsParticipates in orientation of new Team Members to ensure that all new Team Members demonstrate required competencies for their role, and that they receive a complete orientation in adherence with CMS guidanceTracks and reports data required for value based purchasing and ensures accuracy and timelinessCollaborates with Administrator in keeping current with Medicare updates and informing Team Members of all updates to regulationsServes as an on-call manager for the agency on a rotating basis with other agency leadersActs as back up for Home Support Clinical Manager in their absenceMeets department uniform, appearance and grooming requirementsComplies with all OSHA safety regulations, local applicable laws regarding health, safety or environmentEducation and ExperienceBachelor in Science of Nursing or equivalent degreeRegistered Nurse in the Commonwealth of MassachusettsAbility to read, write and understand English3 to 5 years' experience as a Registered Nurse in a professional nursing role in a home health care settingCPR/AED CertifiedNORTH HILL OFFERS:Free mealsFree parkingFree on-site fitness centerState-of-the-art work environmentEmployee referral programHealth & Dental Benefits401(k) with company matchCareer Development TrainingTuition ReimbursementNorth Hill is committed to providing equal opportunity for employment and advancement to all team members and potential team members.
