JOB SUMMARYThe Clinical Manager directs, manages and coordinates home health and hospice clinical services. This is accomplished through leadership and direction of the field staff which may include, RN's, LPN's, Therapists, Social Workers, Chaplain, Home Health Aides and Volunteers who comprise the team directly or through subordinate supervisors. Please see organizational chart for specifics.In accordance with agency policies and standards, the Clinical Manager is responsible for meeting all standards outlined in the Registered Nurse job description. DUTIES AND RESPONSIBILITIES Policy Oversight
- Takes and active role in the planning, evaluation and design of agency programs, policies, and procedures related to clinical requirements.
Performance Improvement
- Oversees and participates in the revisions of the Performance Improvement Programs for Home Health and Hospice, while working with the Administrative Director, Education Director, and Hospice Clinical Director.
- Evaluates performance improvement findings specific to staff performance and other trends to determine strengths and weaknesses. Develops educational and performance plan, as necessary, to ensure improvement occurs with staff.
- Makes clinical recommendations to Directors for program improvements.
Quality Care/Customer Satisfaction
- Provides and assures the provision of quality of care to all patients admitted for service through guidance and coordination of clinical staff.
- Point of contact and one of the responsible leaders for addressing patient complaints, physician concerns, trending and identifying opportunities to improve.
- Builds and maintains positive relationships with stakeholders and community partners.
- Collaborates with Directors to collect feedback regarding the patient's experience, tracks data and helps create strategies to improve the customer experience.
- Ensures care coordination of interdisciplinary patients' services for agency including Nursing, PT, OT, SW, ST, Chaplain and HHA.
Training
- Provides guidance to clinical staff in completion of patient care. Participates in the development of orientation and identifies on-going educational needs of staff in cooperation with Education Director and other agency leaders.
- Collaborates with the Education Director in the development and organization of orientation programs for new employees, volunteers, and students.
- Collaborates with Education Director with areas of education/training including but not limited to: new employees' orientation, on-the-job training, health and safety practices, management development and adaptations to change in policies, procedures, and technologies.
- Must have knowledge of all current OSHA requirements, including the Bloodborne Pathogen Standard, TB Control Plan, HIPPA, Exposure Control Plan and the Emergency Preparedness Plan.
Meetings
- Conducts monthly nurse and aide meetings in collaboration with the Administrative Director.
- Facilitates scheduled case conference and interdisciplinary team meetings for home health and hospice patients (as needed).
- Participates in monthly therapy meetings with contracted therapy companies.
General
- Participates in strategic planning for operational processes.
- Aware of financial impact of standard business operations and makes recommendations for improvements, while enhancing quality outcomes.
- Coordinates and assists in the direction of agency programs to ensure compliance with Medicare, Medicaid, OSHA, ADA, HIPPA and other regulations.
- Has active role in the following: Interviews, screening, hiring, conducting 90-day review, annual evaluations and providing appropriate coaching to direct staff.
- Ensures staff have appropriate competency and are compliant with agency policies.
- Ensures assignments are distributed appropriately and provides instruction for scheduling as necessary.
- Provides clinical guidance and oversight for appropriate on-going care, eligibility, discharge criteria, recertification criteria, transfers, etc.
- Has authority to determine clinical issues as they relate to the clinical direction of the hospice and home care program.
- Maintains referral, admissions, discharge and other related statistics as requested by the Executive Director.
- Accepts Administrator on-call.
- Understands and supports the agencies philosophies, mission and vision.
- Maintains confidentiality of all employee and patient issues.
- Takes accountability for personal growth and development. Maintains and facilitates a positive attitude with his/her employees, peers and leadership.
- Communicates to the Executive Director or with the compliance officer immediately, if at any time they feel that another employee may be violating or are being requested to violate agency policies or government regulations (i.e. Medicare, Medicaid, or other regulatory bodies.) This includes but is not limited to Fraud, Waste and Abuse.

EDUCATION and/or EXPERIENCE
- A graduate of an accredited school of Nursing; Bachelor's Degree Preferred, Ohio License as a Registered Nurse; five (5) years' work experience in home health and preferable progressive supervisory experience is preferred; valid Ohio driver's license, a car in good working order; a good driving record and auto liability insurance as required.
