The Clinical Manager is responsible for coordinating the provision of quality patient care and leadership to the interdisciplinary team including RN's, LPN's, home health aides, medical social workers, and physical, occupational, and speech therapy services. This role facilitates staff development and provides feedback to the Director of Clinical Services regarding patient care services. The Clinical Manager demonstrates flexibility in meeting the agency's needs, assumes delegated responsibilities from the Director of Clinical Services for operational activities that relate to coordination, quality, cost and outcomes of patient care.
Primary Duties:
Operational Oversight: Manage patient caseloads to ensure quality care and adherence to productivity guidelines, organize care conferences, oversee on-call schedules, and foster team collaboration. Additionally, it focuses on effective communication, resolving departmental issues, adjusting staffing levels, training staff on electronic medical records, participating in administrative on-call duties, and supporting the Director of Clinical Services with follow-ups on various concerns.
Patient Care Coordination: Manage the scheduling of patient visits and ensure team members are aware of any special care requirements. This includes leading an interdisciplinary team, helping develop individualized care plans, and ensuring thorough documentation. The Clinical Manager participates in patient care meetings, coordinates discharge planning, and collaborates with various healthcare providers. Additionally, they may take on scheduling duties when needed.
Customer Service: Actively listen to others and being highly aware of both internal and external customer needs. This role requires the ability to ensure patient satisfaction through demonstrated empathy, treating everyone with respect and dignity, effective resolution of conflicts, and openness to questions and feedback.
Performance Improvement: Collaborate with the Quality Improvement Specialist to monitor and enhance patient outcomes and agency performance by using problem-solving techniques, reviewing clinical records for compliance, and ensuring services meet legal and regulatory standards. The role also involves evaluating the effectiveness of Home Health services, maintaining quality standards, and analyzing satisfaction survey results to assess the program comprehensively.
Leadership: Uphold the organization's mission, vision, and values while effectively managing
time and resources. This includes coordinating with the Director of Clinical Services to oversee staff meetings, fostering team collaboration, and delegating tasks. The role also emphasizes efficient resource utilization, maintaining communication with the executive team, and acting as a problem-solving resource. Additionally, it involves modeling professional behavior, empowering the team, supporting professional growth, and ensuring compliance with confidentiality and HIPAA guidelines.
Personnel Management/Supervision: Ensure the nursing and therapy staff adhere to patient care standards. This includes participating in hiring, supervising staff performance, and providing necessary feedback. This role also entails reviewing documentation for accuracy, conducting performance evaluations, setting development goals, and orienting new clinical staff to ensure they complete their skills checklists. Furthermore, this role requires maintaining a professional attitude and effective communication with all agency members and individuals from diverse cultures. It involves relating to all levels of personnel in stressful environments and demonstrating optimism, enthusiasm, and a collaborative spirit to ensure the well-being of patients and employees.
Professional Development: Commit to personal growth, maintain collaborative relationships with peers, and promote the healthcare profession through community involvement and mentorship. This includes completing required competencies to maintain licensure and participating in continuing education activities, sharing knowledge with other staff members.
Secondary Duties:
Intake: Initiate home health services by collecting patient demographic data and entering it into the system, providing referral information to the Insurance Authorization Department, coordinating with the billing department for benefit verification, and ensuring clinical visits meet standards. Responsibilities also include organizing admission visits, managing contracted services, performing home visits as needed, and participating in various meetings and committees.
