The Registered Nurse also works in collaboration with the a multi-disciplinary medical team focused on providing patient centered care by identification of psycho-social needs, continuity of care related to disease prevention and chronic disease management and provides patient education to promote Health Literacy. Specific Job Duties/Responsibilities Demonstrate the WCHS core mission, vision and valuesTo function as primary care Nurse, and assume responsibility of charge nurse duties in order to provide optimal quality of patient careCompletes comprehensive initial admission assessmentCreates Nursing Care plan based on identified or remarkable initial objective assessmentProvides interventions as ordered by the Medical Doctor, Nurse Practitioner, Physician AssistantEvaluates patient response to intervention and promptly notifies Medical Provider of patient condition whenever necessaryObtains specimen collectionGenerates consults to appropriate discipline based on initial admission assessment findingsAssists with Discharge planningProvides patient disease preventable vaccines before dischargeProvides Discharge Instructions related to disease process and home management, and follow-up appointments with their Primary Care Provider to ensure continuity of careDemonstrates ability to float to area of need to meet patient demand (Outpatient, Emergency Room) and/or assists with Emergency Response Preparedness operationsMakes decisions in matters of significanceOther duties as assignedQualificationsQualifications/Skills:Currently licensed as a Registered Nurse in the State of Nebraska or multi state licensure in a nurse licensure compact state.Associate's Degree in Nursing required. Bachelor's of Science in Nursing preferred. STUDENT LOAN FORGIVENESS AVAILABLE.Certifications in Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS) Pediatric Advanced Life Support (PALS)Physical Requirements:Normal to heavy work which includes the ability to lift up to 20-50 lbs.
