We are seeking an experienced and dedicated Clinical Nurse Liaison Nurse to join our team. This role is essential for coordinating care between Blue Ridge Hospice and our community hospitals, ensuring smooth transitions for patients and maintaining continuity of service. The Clinical Nurse Liaison Nurse will serve as a key communicator between the two organizations, patients, and families, while providing clinical care, education, and advocacy.

Position requires 32-40 hours per week, requires some weekend coverage and the need for flexibility for coverage of our hospital programs.

HOSPITAL LIAISON, RN Position Summary

The Hospital Liaison provides a consistent presence at our community hospitals to establish and foster professional relationships with staff and physicians. The Hospital Liaison will engage in continuous and ongoing collaboration with multiple disciplines including but not limited to Social Workers, Discharge Planners, Case Managers, Nurses, Physicians, Nurse Practitioners, and Palliative Medicine. The Hospital Liaison is ultimately responsible for coordinating referrals of new patients to BRH. TheHospital Liaison ensures that patients and/or families of patients who have been referred are provided education regarding Blue Ridge Hospice services and philosophies through informal visits.

HOSPITAL LIAISON, RN Essential Duties & Responsibilities

Documentation & Communication:

  • Ensure accurate and timely documentation of care provided and the patient's/familys response, adhering to organizational policies.
  • Facilitate continuity of service across care settings through effective information management, including both verbal and electronic communication.
  • Provide accurate clinical documentation in both the Blue Ridge Hospice electronic medical record (EMR) and hospitals EMR.

Collaboration & Coordination:

  • Collaborate with hospital staff regarding potential patient referrals to Blue Ridge Hospice.
  • Coordinate admissions between Blue Ridge Hospice Intake and community hospitals, providing accurate information and reporting for the Hospice Physician to determine hospice eligibility.
  • Order Durable Medical Equipment (DME) as needed for patients and coordinate with the Intake Department.

Patient Care & Advocacy:

  • Conduct supportive visits to patients already established with Blue Ridge Hospice who have been admitted to community hospitals and communicate the patient's status and needs to the primary care team.
  • Meets with caregivers and/or family members to share information regarding hospice services, lead family meetings, and obtain admission consents.
  • Complete Blue Ridge Hospice admissions within the hospital setting and act as a patient advocate.
  • Implement nursing interventions to relieve pain and other symptoms of life-limiting illnesses, delivering care within the scope of nursing practice and in accordance with accepted standards.
  • Participate in holiday coverage, rotating at least 1-2 major or minor holidays per year among staff to ensure consistent service delivery.

Education & Support:

  • Provide formal and informal education related to hospice philosophy and techniques of pain and symptom management.
  • Articulate the essential components of the hospice benefit as administered under Medicare and other payers.

HOSPITAL LIAISON, RN Qualifications/Experience

  • Current Registered Nurse (RN) license from the State of Virgina. BSN preferred.
  • Excellent oral and electronic communication skills.
  • Proficient computer skills and electronic medical records required.
  • Must be able to travel throughout the territory as needed.
