Position Overview:The Bon Secours Program for Diabetes Health is seeking a Clinical Nurse Specialist (CNS) to join our dynamic team. In this pivotal role, you will work across all four Richmond-area hospitals to provide expert diabetes care to hospitalized patients. You will collaborate closely with multidisciplinary teams, contribute to the growth and development of healthcare providers, and lead efforts to implement evidence-based diabetes care standards.

Essential Job Functions:

  • Deliver Expert Diabetes Care & Self-Management Training: Provide state-of-the-science diabetes care, using evidence-based guidelines and clinical judgment. Conduct diabetes self-management training (DSMT) for patients to promote ongoing care and management.
  • Advanced Nursing Assessment & Care Planning: Perform advanced nursing assessments, develop individualized care plans, and execute evidence-based protocols in collaboration with multidisciplinary teams.
  • Collaboration & Education: Work alongside a diverse team of healthcare professionals to provide comprehensive care. Lead educational initiatives and training programs for staff to ensure consistent, evidence-based diabetes management across the system.
  • Mentorship & Clinical Development: Mentor nurses and other team members in developing their knowledge and skills in diabetes care. Facilitate the integration of best practices for diabetes management throughout the hospital system.
  • Program Development & Implementation: Play a key role in shaping and expanding the hospital's diabetes care programs. Work collaboratively with leadership to design, implement, and refine protocols, policies, and strategies that enhance diabetes care delivery.
  • Compliance & Record-Keeping: Ensure adherence to national standards of care (ADA, AACE) and regulatory requirements (CMS, Joint Commission). Maintain accurate documentation in the electronic health record (EHR) to meet clinical and productivity standards.

Qualifications:

  • Education: Masters degree in Nursing with a focus on diabetes care, or equivalent clinical expertise.
  • Licensure/Certifications:
    • Advanced Practice Registered Nurse (APRN) with certification as a Clinical Nurse Specialist (CNS).
    • Certification in Advanced Diabetes Management (or eligibility to obtain certification within a defined time frame).
