DescriptionTITLE: Assisted Living Clinical Nurse SupervisorREPORTS TO: Housing Manager$1,000 Sign on bonusJob Summary: The role of the CNS is to ensure that the health and safety of all residents are met throughout the facilities clinical services. The CNS will ensure that that the facilities are following current regulations and standards, through organizing, planning, directing, supervising, coordinating and evaluating the facilities clinical services. Must be able to relate professionally with residents, staff and family members, display a positive work attitude and be able to work as a team with other staff members. May be engaged in direct resident care. Qualifications: Must be registered and licensed with the State of Minnesota as an RN and must maintain licensure with at least 3 years of long term care experience and 3-5 years of leadership experience. Must have a valid driver's license. Must have working knowledge of long- term care services, rules and regulations related to 144 G Assisted Living Licensure. Must have excellent judgement and decision-making abilities and problem-solving skills. The CNS must have basic computer skills and the ability to use electronic health records appropriately. The CNS is also required to have an ability to read, write and speak English, and complete and pass a state required background study form which checks for certain criminal convictions or serious/recurring maltreatment.General Responsibilities and Specifications:
  • Ensure residents have up to date care plans and service plans.
  • Monitors and maintains resident charts and clinical progress notes.
  • Communicates to physicians, other personnel (county caseworkers), and designated representatives (per resident preference) changes in resident's needs or conditions.
  • Facilitates coordination of resident services with other service providers within the current service limitations as described in the UDALSA.
  • Ensures and participates in all aspects of medication management services for applicable residents.
  • Ensures all physicians orders are signed and implemented timely.
  • Ensures completed assessments per regulations.
  • Assist with admissions as directed.
  • Ensures that delegated nursing tasks are followed properly and discuss any needed corrections, errors, etc., with staff.
  • Worksas a team player with all members of the resident's care team.
  • Ensures safe practices of medical equipment being used in the building.
  • Ensures that staff clearly understand and follow all resident care procedures.
  • Shares resident and family concerns with pertinent staff if appropriate.
  • Assists in the development of policies and procedures and implementation of them.
  • Attends routine staff meetings and trainings.
  • Assist in orientation and on-going supervision of licensed and unlicensed personnel.
  • Supervises medication practices and medical equipment being used in the building.
  • Ensures compliance with regulations and is available for federal and state licensure surveys.
  • Name and license on file with the MDH licensing division.
  • Ensures, in conjunction with the LALD, a written staffing plan is in place, that provides adequate level of direct care staff to meet the current resident needs.
  • Workswith staff to assure emergency procedures are in place, practiced, and followed through in the event of an emergency.
  • Is flexible and willing to expand and adapt to changes in the industry.
  • Promotes a positive, team approach and a healthy work environment.
  • Ability to deal tactfully and use good communication and customer service skills with staff, residents, families, and professional encounters.
  • Scheduled on call per rotation. Is available for on-call medical questions and/or emergencies via phone. Occasional need to assist in house when on call.
  • Obtain required education upon hire and annually thereafter to meet regulatory requirements.
  • Must obtain 8 hours Dementia Training within 80 hours of start of employment- ALDC Licensed Facilities only
  • Must obtain 8 hours Dementia Training within 120 hours of start of employment- AL Licensed Facilities only
  • Obtain required Dementia Training annually thereafter
  • Understand and adhere to the Vulnerable Adult and abuse policy, Bill of Rights and HIPPA.
  • Apply principles of good body mechanics when lifting and moving objects. Use transfer techniques consistent with proper body mechanics.
  • Participate in disaster planning drills (fire, tornado, severe weather, etc.) when on duty.
  • Maintain awareness and adheres to use of safety devices according to the care plan and suggests their use when appropriate.
  • Practice fire safety and accident prevention procedures, including participating in emergency drills when on duty.
  • Workswith staff to assure emergency procedures are in place, practiced and followed through in the event of an emergency.
  • Follow all policies and procedures governing the operation of Minnewaska Community Health Services.
  • Other duties as assigned by Housing Manager, Regional Assisted Living Director, Vice President of Assisted Living Operations and CEO.
