Job Description:
POSITION SUMMARY:The Patient Navigator will assist clinical team members by performing a variety of duties for patients within the patient focused care system and serves as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with scheduling additional imaging procedures ordered for continuation of care within DHR Health. Facilitates patient appointments for breast imaging procedures.POSITION EDUCATION/ QUALIFICATIONS:
  • Graduate of accreditedAssociates orBachelor'sDegreeprograminnursing,radiology, or health-related
programrequired.
  • Current license in the State of Texaspreferred.
  • Current BLS certification required.
  • BreastNavigator Certificationwithin24months required, if navigating Breast Imaging.
  • CT Lung Screening Certification within 24 months required, if navigatingtheCT Low Dose Lung Cancer
Screening Program.
  • Must be able to be sensitive to cultural and bilingual issues.
  • Good written and verbal communication skills required.
  • Ability to read, write and speak Englishrequired, Spanish preferred.
  • Ability to communicate clearly and concisely with all levels of nursing, administration, and physicians.
JOB KNOWLEDGE/EXPERIENCE:
  • Work experience and knowledge in corresponding imaging (i.e. breast imaging, CT) preferred.
  • Must have excellent customer service skills.
  • High degree of competency/experience in general.
  • Requires good communication and organizational skills.
  • Requires reasoning ability and strong decision making skills.
  • Requires excellent leadership and interpersonal skills.
  • Excellent presentation skills, preferred.
  • Ability to implement professional and community based education programs.
  • Microsoft Office competency required.
POSITIONRESPONSIBILITES:
  • Promotes the facility mission, vision and values by effectively communicating them to others.
  • Considers mission, vision and values in developing services, standards and practices.
  • Assist patients inscheduling their additional imagingbased on the radiologist's recommendations and physician's orders.
  • Serve as an essential link between patients and all other care providers and assist to facilitate appointments for consults and support servicesasneeded.
  • Develop patient educationinformationand tools.
  • Follow patients throughout theimagingcare continuum.
  • Work with staff to collect data, track outcomes, and support strategicplanning processes.
  • Deliver quarterly report todedicatedcommitteeswhich demonstrate outcomesand performance improvement activities.
  • Adhere to established productivity benchmarks.
  • Responsible for outreach efforts to establish and maintain positive working relationships with keycustomers (physicians, office staff, diagnostic staff, nurses, radiology staff, social services staff,and radiation oncology staff, etc.).
  • Attend assignedtrainings and meetings.
  • Other duties as assigned.
