Job Title: Clinical Pharmacist - Cardiology/Anticoagulation (FTE & Per Diem)

Location: White Plains, New York

Department: Pharmacy

Employment Type:

  • Per Diem: Every other Weekend 8:00 AM - 4:00 PM
  • Full Time: Monday to Friday 12:00 PM - 8:00 PM

Must-Haves:

  • PharmD degree from an accredited school of Pharmacy.
  • Current New York State Registration.

Position Summary

Under the general supervision of the Director of Pharmacy Services, the Clinical Pharmacist will be responsible for managing rational antibiotic therapy or the care of critical/high-risk patients in the Cardiology and Anticoagulation departments. This role involves collaboration and consultation with other healthcare professionals, as well as providing education and training related to the therapeutic use of antibiotics or other medications.

Essential Functions and Responsibilities:

  • Adherence to Standards: Understand and adhere to the Performance Standards, Policies, and Behaviors of the hospital.
  • Clinical Management: Improve monitoring and management of clinical pharmacy services related to antibiotics, critical care, or high-risk patients, ensuring quality and cost-effective patient care.
  • Clinical Leadership: Provide clinical leadership and education to pharmacy staff to promote clinical pharmacy development.
  • Collaboration: Work collegially with colleagues to achieve organizational objectives, fostering teamwork, shared goals, and new ideas.
  • Innovative Approaches: Generate creative approaches to improve performance, service, and quality, involving appropriate stakeholders in decision-making.
  • Personal Responsibility: Take personal responsibility for work activities and actions, following through on commitments, and acknowledging mistakes.

  • Education & Experience Requirements:

    • Graduate from an accredited school of Pharmacy with a PharmD degree.
    • Current New York State Registration.
